MMS® Remote CCG Proactive Care Pharmacist Support Service

The Leicester, Leicestershire and Rutland (LLR) CCGs have commissioned Medicines Management Solutions Ltd (MMS), to provide remote Clinical Pharmacist Support to support surgeries with the review of patients identified at high risk within the UCL Proactive Care Framework. The CCGs have shared the following insight;

Due to patient safety concerns, we have commissioned additional pharmacist support through Independent Providers to support you in completing reviews for your highest priority patients for Hypertension, AF and Asthma. This work will be for the next 7 weeks up until March 31st.

As part of the resilience built into this offer the MMS workforce will work remotely and to work in and out of core hours, hence the service has been commissioned to maximise capacity 7 days per week 8am to 8pm.

Each MMS Pharmacist will be deployed with a specific schedule and caseload – please complete and submit this form to support engagement and approve the scope of service.
