DECLARATION AND COMMITMENT FORM

To be signed by all staff participating in any of the children's programs and events at WVC. A copy is to be kept by the staff member and on church file.

In accepting this position I declare that I:

• accept the leadership of the pastor(s) and Child Safe Leader at all times
• will work within the procedures as laid out in the WVC Child Safe Policy and Procedures including the WVC Staff, Leader and Volunteer Code of Conduct (Appendix 3) and any subsequent changes which I will be notified of as they occur.
• will be a role model to those in my team and provide any guidance and support to them as needed
• will work to the best of my ability and within all best practices for WVC children’s programs and events
• will work in a cooperative and supportive manner with all staff and volunteers involved in WVC children’s programs and events
• have not been accused or convicted of any offence involving physical or sexual abuse of children or young people.
• will attend all required training
• will attend all required meetings unless I am unavailable due to illness or family/work-related commitments. When I am not able to attend a meeting, I will notify the Pastor(s)/Child Safe Leader and commit to obtaining any information discussed at the meeting
• understand that if a complaint be levelled against me while engaged in any WVC activity the police or other legal authority will automatically be asked to investigate
• understand that if I come under investigation for abuse I will be suspended from my tasks/roles with vulnerable people and that if found guilty my position will be terminated
• can advise the pastor(s) or Child Safe Leader verbally or in writing, if at any time I wish to withdraw from my volunteer role at WVC, of my intent to withdraw from this ministry.
• I understand that my declaration, application and any other documents will be kept on file for evidence of compliance indefinitely.
• all information given by me is true and correct.
