    Below, please explain the nature of your concern and provide as many details as possible. We will review the situation impartially under our policies. It is most helpful if you provide your name and contact information. While this is not required, it will assist us in addressing the matter. We may contact you for further information or give you a general update on how HEPNet has addressed the matter. We will share your name with others only on a “need to know” basis. HEPNet prohibits retaliation against anyone who raises a concern in good faith. We place utmost importance on the obligation against retaliation.

     

    To support your anonymity, you can unselect an internal party below if that person receiving a copy of this report could be a conflict. The report must go to our external consultant and at least one internal party as well.
