I am so glad you are interested in becoming a part of my clientele! I created this form as a way to analyze where your current hair is and help with finding what services to choose for your first visit in the salon with me to achieve your hair goals/needs.

After completing this form I will contact you back within 24 hours Tuesday-Saturday with what services are best to book with me.

If you need a more in-depth analysis or have initial questions prior to completing this form I am more than happy to discuss them with you via email. If not, lets get started!
