  • If your travel destination requires that your passport number be listed on your results documentation, please email us the date of your test, your full name, date of birth and passport number to jcomm.covidtest@gmail.com. NOTE: If you don't include the date of your test, we may not be able to update your documentation.

We urge you to reach out to your primary care physician to obtain information on Monoclonal AB Therapy. **NEW** Hatzalah of Baltimore in conjunciton with the federal government has now opened a therapy clinic making this available to any high risk individuals who test positive. You will receive information by email after you submit this form.

  • HIPAA Authorization and Waiver of Liability

    The undersigned has requested a COVID-19 and/or other respiratory pathogen diagnostic test for the undersigned and/or the minor child of the undersigned at the ACCU Testing location located at 2010 Linden Ave. Silver Spring, MD 20910 or other location. 

    The undersigned hereby releases from any liability ACCU Testing, all of the individuals performing the testing swabs, the Torah School of Greater Washington, and any other entity involved in hosting the ACCU Testing COVID-19 Testing, and all of their respective officers, directors, employees and agents.

    The undersigned authorizes ACCU REFERENCE MEDICAL LABS and ACCU Testing, Dr. Yosef Levenbrown, Dr. Julian Jakobovits and any other healthcare provider I designate as a recipient of the protected health information referenced herein.

    Effective Period: This authorization for release of information covers the period of healthcare from 8/1/2020 TO 12/31/2022.

    Extent of Authorization: The undersigned authorizes the release of his/her complete health record.

    This medical information may be used by the person the undersigned authorizes to receive this information for medical treatment or consultation, billing or claims payment, or other purposes as the undersigned may direct.

    The undersigned understands that the undersigned has the right to revoke this authorization, in writing, at any time. The undersigned understands that a revocation is not effective to the extent that any person or entity has already acted in reliance on this authorization or if the undersigned's authorization was obtained as a condition of obtaining insurance coverage and the insurer has a legal right to contest a claim.

    The undersigned understands that information used or disclosed pursuant to this authorization may be disclosed by the recipient and may no longer be protected by federal or state law.

                  This is for regular rapid antigen tests, not rapid PCR. This test is NOT billed to isurance. Results in 4-5 hours.

                  At-Home Testing - Home Visit and One Standard PCR Test

                  This fee is for a home visit and also includes 1 standard pcr test. Additonal standard pcr tests are $15 which can be paid at the time of the test. Rapid tests are the prices listed ($135 for rapid pcr and $40 for rapid antigen). Note that our service area is within a four mile radius of our office. Other areas may be available for additional fee. CANCELLATION POLICY: Rapid test fee's are refundable. At-home service fee is not refundable if cancelled on day of scheduled test.
