Part 2: Statement of Understanding
Please review the following terms listed below. Note that your signature at the end of this document indicates that this "Statement of Understanding" section has been read, understood and you are in agreement with the terms and conditions as outlined within this document.
A Little Zen will issue an invoice via email directly to each registered participant for their individual cost to attend the program. Please note that a NON-REFUNDABLE registration fee of $50.00, required to secure your spot in the program and will be used towards your weekly installments (if applicable). Failure to remit payment will result in no formal registration by the participant. Please be advised that payment may be offered via e-transfer to Zenia@alittlezen.ca or via credit card (please contact us directly to arrange for credit card payment). Receipts will be provided following each session/program for you to submit to your benefits program for reimbursement.
Please check with your extended health benfits plan for coverage for "Social Work Services (MSW or RSW)"; Counselling" or "Psychotherapy".
Please be advised that if you must cancel your participation in the program after registering and remitting payment, at least 5 business days of advanced notice is requested. The $50 registration fee is NON-REFUNDABLE.
If you are unable to attend a scheulded session, 24hr notice is required, failure to provide 24hr notice, cancel the day of, do not attend or are more that 15 minutes late without prior communciation, you will be charged the full session rate.
Your presence in the group is highly important. We ask that you make a commitment to attend every session. For multi session progrmas, each session builds on the previous, and attending regularly generates the most benefit for you and your fellow group members. If you are unable to attend a session, please notify the group facilitator and/or bring this up during the group session well in advance.
We are going to do a lot of different things - explore concepts, engage in experiential exercises and mindfulness exercises, articulate what we really want in life, and encounter thoughts and feelings in new ways. Do your best to put your whole self into the experience. In order to encourage everyone's participation in the group and provide everyone with the opportunity to do so, we will often go around the group to hear everyone's reaction to something we may be discussing. The more you put into the group, the more you are likely to get out of it.
What to Expect:
Group sessions will consist of your facilitator presenting certain skills and concepts, a lot of group discussion, mindfulness exercises to cultivate presence, and some experiential exercises.
We will be talking about approaching thoughts and feelings in a very different way from the way most of us have been taught. Therefore, you may be a little confused or uncomfortable at times, especially in the beginning. We ask that you approach this confusion or uncomfortablity with openess and willingness. It is a normal part of the process, and you do not have to understand everything perfectly to benefit from it.
Most of the strategies, experiences, and techniques of this group build upon one another, and it sometimes takes a while for their benefit to become obvious. "Improvement" happens at its own pace and often continues after the program has ended.
A safe environment is of utmost importance and will be created and maintained by your facilitator, yourself, and your fellow group members. The primary ingredients of a safe environment are mutual respect and a chance to create trust. Please be respectful of one another as we share and discuss with each other. Please refrain from insulting, criticizing, or interuppting one another. Your group facilitator will do their best to maintain a respectful and safe group environment so that everyone
feels comfortable to participate and share their thoughts and experiences. If anybody becomes disruptive to the group and its members or appears to be intoxicated, they may be asked to refrain from participating or to leave.
As we get to know one another, people will begin to share personal details of their lives, feelings, and thoughts. Our normal reaction when someone is suffering is to try and help them get away from their pain. We do this by urging them to "look on the bright side" offer solutions, or telling them "it's going to be okay". We call this "rescuing". We are going to try to NOT do this in-group. Not because it is a bad thing, but because it is a strategy for controlling our feelings. In this group, we will attempt
to move toward pain without letting it be in charge of us. This may not make complete sense right now, but we will talk more about it as we proceed with the group.
Emergencies - Your facilitator is not able to offer crisis support during programs.
While your facilitator will do their best to provide added support when needed, in case of an emergency or crisis, please contact 911 or an appropriate crisis line, or go to the nearest emergency department.
Consent to Treatment using Yoga:
I acknowledge that some of the therapeutic approaches may include yoga postures, breathing exercises and physical movement and with any exercise there is a risk of injury. I have consulted with a physician prior to embarking in any physical yoga practice facilitated by your A Little Zen facilitator and release any and all liability for injuries that are directly related and proximately caused by their professional negligence. I voluntarily assume a certain risk of injury.
I am aware it is my choice to participate in yoga, movement or outdoor activities with your A Little Zen facilitator/s and when accepting of participating in any of these activities I will be responsible for the following:
· Breathe smoothly & continuously as I move & stretch
· Do not strain to attain any position
· Work gently, respecting my body’s abilities & limits
· Do not perform postures or movements that are painful
