  • The Racism is a Public Health Crisis Implementation Tool lists the organizational practices identified in the Racism is a Public Health Crisis Declaration. Organizations must identify an Executive Sponsor, a responsible staff person and an experienced, internal cross-functional team to complete this tool and to implement the selected practices.

  • There are three stages of implementation listed for each practice:

    1) Currently Implementing

    2) Planning to Implement

    3) Not Planning to Implement

    Steps to complete Assessment:

    1. Select your organization’s stage of implementation for each practice.

    2. After you have selected your organization’s stage of implementation add the timeframe to each “planning to implement” practice selected.

    3. After you have identified the timeframe for implementation write down a few concrete steps identifying how your organization will implement the practices selected. Consider your objectives, challenges, staffing and the resources needed to support achievement.

    4. This tool must be reviewed and updated every 30 to 90 days.
