For Miss Minnesota and Miss Minnesota's Teen Local & State Scholarship Recipients
Dear Scholarship Recipients,

This online Scholarship Request form MUST be used by Miss Minnesota and Miss Minnesota's Teen participants that have been awarded scholarships at the state competitions.

Scholarships may be requested for Tuition and Student Loans.

MISS Candidates have ONE year from the time they are awarded the scholarship to request their scholarship or they will be forfeited. TEEN Candidates have ONE year from the time they graduate high school (or equivalent) to request their scholarship or they will be forfeited. No Extensions.

All requests must be complete and provide enough lead time prior to the payment due date for us to approve, process and mail scholarship payments.

REQUIREMENTS

Please be sure you have all the necessary documentation below before starting or submitting the online scholarship request. Incomplete or incorrect submissions will be declined and you will need to begin a new scholarship request.

TUITION/HOUSING REQUIRED DOCUMENTATION

Requires a current detailed tuition bill/statement clearly showing the information below. Housing will only be covered if on campus through the college/university. Off campus housing will not be covered.

If submitting school bill...
- Must be a full school bill, not screenshots
- Must show due date with at least three weeks to pay
- School name, remittance instructions and address
- Scholarship recipient's name and student ID
- Details of charges for tuition, housing etc...
- Transcript
If submitting school statement...
- Must be a full statement, not screenshots
- Must show date within the past 30 days
- School name, remittance instructions and address
- Scholarship recipient's name and student ID
- Details of charges for tuition, housing, etc...

STUDENT LOANS REQUIRED DOCUMENTATION

Requires a current bill from the lending institution, a copy of the promissory note and a copy of the transcript showing the completion of the coursework.

- Must be a full bill and not screenshots
- Must show due date with at least three weeks to pay
- Lending institution name, remittance instructions and address
- Scholarship recipient's name and account number
. Promissory Note
- Must show Scholarship Recipient as the primary payer of the loan
- Only need the first couple of pages showing the specific loan details
. Transcripts
- Transcript to show completion of coursework
- Copies of transcripts accepted, does not need to be official sealed transcripts.

For Miss Minnesota and Miss Minnesota's Teen State Scholarship Recipients
INSTRUCTIONS

The form must be completed by the Scholarship Recipient and not family members.

We will no longer accept paper scholarship forms or documentation sent by postal mail or attached to an email. All Scholarship requests must be submitted using this online form process.

PRIORITY OF USE

If you were awarded scholarship funds at the LOCAL level and the STATE level, you must request and receive all LOCAL scholarships before receiving STATE scholarships.

STATE SCHOLARSHIP REQUESTS

Submissions for State Scholarships will automatically be forwarded to the Minnesota Educational Scholarship Fund, Inc. for review, approval and processing. The Minnesota Educational Scholarshiop Fund, Inc. will confirm with the Local Programs that all available local funds have been exhausted. The Minnesota Educational Scholarship Fund, Inc. may choose to process payments from their scholarship account or to be processed by the Miss America Foundation on behalf of the Minnesota Educational Scholarship Fund, Inc.

NATIONAL SCHOLARSHIP REQUESTS

Submissions for National Scholarships will automatically be forwarded to the Minnesota Educational Scholarship Fund.Inc. for review and confirmation that the ducumentation meets the national requirements.

The Minnesota Educational Scholarship Fund, Inc. will verify that the recipient has utilized all Local and State Scholarship funds. We will then provide the recipient with a confirmation letter that the recipient will need to complete the National Scholarship Request.

At the same time, we will provide the instructions for the scholarship recipient to submit the request using the NATIONAL online process for MISS and TEENS. The Minnesota Educational Scholarship Fund, Inc. is no longer able to submit the request on the recipient's behalf. It must be submitted directly by the recipient.

AUTOMATED NOTIFICATION

When you submit your online scholarship request, you will receive an email confirming we have received your submission. As your scholarship request is being processed, you will receive additional emails with the updated status information at the bottom of the email.
