During children's programs, activities and events photos and videos may be taken.

This form is seeking your permission to take and use photographs/videos taken at WVC or related events of your child/ren.

Photos/videos may be used for Church displays, on the WVC website, WVC publications (eg. Welcome Pack, newsletters, promotional material), WVC Facebook (closed group), WVC Facebook (public page) or the WVC Instagram account (public

Pictures/videos will not be used for any purpose other than those listed above and for which permission has been granted unless further consent is given eg. Newspaper media.

Every endeavour will be made to adhere to the above permission given for your child, however we are unable to control the use of images of your child which have been taken by others on their own personal devices.

If you have more than one child covered by this form please speak with the Leader to get an additional copy of this permission slip if you do not wish all children to be covered under the one approval.
