  • COVID-19 Liability Release Waiver for Arts on the Horizon

    If my child has a temperature above 100 degrees or is experiencing other symptoms (fever, chills, cough, shortness of breath, sore throat, fatigue, muscle or body aches, new loss of taste or smell, nausea, vomiting, or diarrhea), we will keep them home and I will contact Arts on the Horizon.

    I am aware of the existence of the risk related to my child’s participation in the Arts on the Horizon camp may result in illness such as, but not limited to COVID-19. I am fully and personally responsible for the safety of members of my household during the Arts on the Horizon camp, and I recognize that members of my household may be at risk of contracting COVID-19.

    With full knowledge of the risks involved, I hereby release, waive, discharge Arts on the Horizon, its board, officers, administrators, teachers, staff and assigns from any and all liabilities, claims, demands, actions and causes of action whatsoever, directly and indirectly arising out of or related to any loss, damage, injury or death that may be sustained by any member of my household, including my child, related to COVID-19 while participating in the Arts on the Horizon camp that may lead to unintentional exposure or harm due to COVID-19.

    I agree to indemnify, defend, and hold harmless Arts on the Horizon from and against any and all costs, expenses, damages, lawsuits, and/or liabilities or claims arising whether directly or indirectly from or related to any and all claims made by or against any members of my household due to injury, loss, or death from or related to COVID-19.

    By signing below, I acknowledge that I have read the foregoing Liability Release Waiver and understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; that I have been sufficiently informed of the risks involved with participation in the Arts on the Horizon camp and give my voluntary consent in signing below as my own free act and deed.

  • By registering your child, you are agreeing to the following:

    • The information I have provided on this form is correct as far as I know

    • If you wish to cancel your camp registration, you must notify Arts on the Horizon via email at least 4 weeks prior to the start of the registered session in order to receive a tuition refund in the form of an account credit to be used toward future Arts on the Horizon education classes or camps. If you cancel your registration less than 4 weeks prior to the start of the camp, you will receive 50% of your registration in the form of a credit.

    • Low Enrollment/Cancellation Policy: A minimum enrollment is required to run each program session. Sessions with low enrollment prior to the start date may be subject to cancellation. Any registrations that are cancelled due to low enrollment will be refunded in full.

    • Although every effort is made to provide a safe environment, I recognize there is always a risk of accident or injury. I agree to be responsible for any medical expense incurred from illness or injury as a result of my child's participation in Arts on the Horizon’s classes or camps. I understand that children are expected to be covered by their own, or their parent’s or guardian’s, insurance. Accordingly, I hereby release Arts on the Horizon from any and all liability, claims, or damages arising out of my child’s participation at Arts in the Horizon’s classes or camps.

    • Additionally, I authorize Arts on the Horizon to administer necessary treatment for my child. I hereby further release Arts on the Horizon from any and all liability, claims, or damages that may result from such administration of treatment.

    • I have read, understood, and agree to the information listed above.
