Catflap Access Acceptance Form : 

I agree that as my cat/s have access to a catflap there is a risk of incidents occurring whilst not under the care of The Four Paws Agency and anderstand that all terms and conditions remain the same: If your cat/s don't have access to a catflap then please put N/A
