I, acknowledge that I am voluntarily participating in karate classes and activities at HP SHOTOKAN LLC. I understand that participation in karate classes involves physical activity and certain inherent risks, including but not limited to injury, illness, or property damage.

In consideration for being allowed to participate in karate activities, I, on behalf of myself and my heirs, executors, administrators, and representatives, hereby release, waive, discharge, and covenant not to sue HP SHOTOKAN LLC, its owners, instructors, staff, and volunteers from any and all liability, claims, demands, actions, or causes of action for any injury or damage resulting from my participation in karate activities.

I understand and assume all risks associated with karate, including but not limited to falls, contact with other participants, equipment, and any negligence on the part of HP SHOTOKAN LLC or its staff.

I certify that I am in good physical condition and have no medical or physical conditions that would prevent my participation in karate activities. If I have any concerns about my ability to participate, I will consult with a medical professional before participating.

I understand and agree that this release of liability is intended to be as broad and inclusive as permitted by the laws of the state in which HP SHOTOKAN LLC operates. In the event that any provision of this waiver is held to be invalid or unenforceable, the remaining provisions will continue in full force and effect.

I have read this release of liability waiver and fully understand its terms. I understand that I am giving up substantial rights, and I sign it voluntarily without any inducement or assurance of any nature.

30 DAY NOTICE OF CANCELLATION OF AUTOMATIC PAYMENT. I will give HP Shotokan a 30 day notice if I need to modify or cancel my automatic payment. If I fail to do so, I will be responsible for the following months membership payment.

SHOTOKAN (2 DAYS PER WEEK)

This monthly membership includes UP TO 2 classes per week.. This membership DOES NOT include competition team class. Must give 30 day notice before cancelling. NO REFUNDS.

Family Special - (3-4 Family Members)

This monthly membership is for groups of 3-4 family members. It can be siblings, parent and child. Only valid for General SHOTOKAN clases. Must give 30 Day notice before cancelling.

Competition Team 2 Days

You MUST be a current HP Shotokan student to be apart of the competition team. This iis for serious competitors ONLY. Competitors will get tournament prep in kata and Kumite for upcoming events. This includes at least 2 days per week of additional training.

This is for 1 student.this is for unlimited classes but does not include competition team class. This will automatically be charged semi-annually. NO REFUND if student decides to quit before 6 months are up. NO REFUNDS

This is for 1 student. This membership includes unlimited classes, but NOT including competition team class. Membership will be charged in an annual basis. NO REFUNDS. No refunds includes if student decided to quit before 1 year contract is finished. NO REFUNDS
