The field trips of the State University of NY, College at Oneonta Outdoor Program include canoeing, kayaking, hiking, backpacking, cross country skiing, snowshoeing, downhill skiing, snowboarding, mountain biking, caving, rock climbing, whale watching, horseback riding, among other activities, often in remote areas. They also involve traveling on major highways and State or county road systems. These trips require participation in activities that are by their nature physically and mentally demanding. With this in mind, all participants must be free of medical or physical conditions that can create risk to themselves or the other members of the trip group who depend upon them. Medical assistance may not be immediately available. Each person must be willing to comply with College rules and regulations to aid in ensuring the safety of all trip members. With your signature below, you indicate that you understand the nature of the trip and the fact that not all of the stresses and hazards of the trip can be foreseen. You understand that reasonable precautions will be taken to maintain the safety of the trip participants and, further, that you have a personal responsibility to comply with the trip rules and safety policies. You are expected to exercise good judgment and reasonable care that a prudent person would use under the circumstances to maintain the safety of the experience for yourself and others. Failure to do so could result in injury or death to yourself or other members of the trip group.

Your signature below indicates that you understand the information on this form and the content of associated discussions. Further, you indicate that you freely consent to participate in the trip under those conditions.

1. I am the below named participant who wishes to participate in the State University of NY College at Oneonta Outdoor Program.

2. I declare that I have been properly informed of the potential risks of physical injury inherent in an activity of this nature and agree to follow all directions for the prevention of such injury to others or myself and agree not to seek recourse against the College for injuries to myself for which the college is not negligent.

3. I also agree to hold harmless and indemnify the State University College at Oneonta and its agents and employees for the payment of damages arising out of actions commenced by third persons alleging personal injuries, property damage or death arising from my acts, which are negligent, malicious or intentional.

4.I have read and understood the foregoing statement and sign it with knowledge of its meaning and content.

Equipment is available on a first come first serve basis. Reservations may be made up to 7 days in advance. Trips through the outdoor programs or associated clubs may reserve up to 3 months in advance. Reservations can be made in the Outdoor Programs office, by phone, or through our online reservation form. Full payment is due at the time of pick up. Your reservation will be cancelled if your order is not picked up on the date reserved. If you no longer wish to rent equipment, please let our staff know ASAP so we can let others use the equipment.

No equipment will be considered returned until it is inspected by our staff. Returning equipment must be done during normal office hours unless other arrangements have been made. Plan accordingly so that returns are during office hours which are posted in our office and online. There are no refunds for early returns.

If equipment is returned past due, each item will be subject to a late fee that accrues daily. The late fee is equivalent to 2x the daily rental price. If no rental fee was associated with the item, the late fee will be $1.00 per item per day late.

DO NOT CLEAN ANY CLOTHING! Some clothes must be washed with special detergent to prevent damage to the product.
 
Other rental items must be clean (outside of normal wear & dirt) upon return. A $10 per item cleaning fee will be charged for equipment that must be cleaned by our staff. If you are unsure of how to properly clean an item, please ask the OP staff. If there is a mess that is unable to be cleaned or causes permanent damage to the product, damage or replacement fees may be charged.

If an item is lost or stolen, the renter is responsible for all costs to replace the item(s). Product replacement costs can be found posted in the Outdoor Programs office.
 
If an item(s) is returned with damage beyond normal wear & tear, the renter will be responsible for either paying for the repair costs, or replacement of the product (whichever is less).

Equipment should only be used for its intended purposes. All activity related safety equipment should be worn and properly adjusted while using rented items (helmets, PFDs, etc.).

It is responsibility of the renter to know how to use the requested equipment. The renter may ask a staff member for clarification about the proper use of equipment; however, such an explanation is not a substitute for proper training from a certified instructor.
 
It is the responsibility of the renter to check all equipment prior to leaving the rental desk, and the renter must agree to accept the equipment “as is.”

I, the renter, agree to pay for any damages or loss of equipment, and hereby assume all responsibility for all risk of injury, accident or loss which may occur while using rented equipment. I hereby agree to hold harmless, release and forever discharge SUNY Oneonta, its employees, and the State of New York from all claims and demands which the undersigned or any representative of the undersigned may have. The terms of this release will serve as a release and assumption of risk for my heirs, executors, and administrators for all my family members.
 
By signing this form, I certify and acknowledge that I have read the above equipment rental policies and procedures for the SUNY Oneonta Outdoor Programs and agree to abide by these guidelines. This agreement is valid for one year after signing.
