RHS Dance Guest Application

RHS Application for Dance Guests

This form is REQUIRED for admission to a Rochester High School dance for all guests who are not a current student of Rochester High School, including former students and alumni. Guests will not be permitted to enter the dance if they do not have this form on file prior to the day of the dance. One guest per RHS student is permitted.

Guests who currently attend Adams, Stoney Creek, ACE, or the International Academy DO NOT have to complete this guest form, but are required to show their school student ID or driver's license at the door. Guests who attend another school in our district will not be permitted to enter the dance without showing ID.

Anyone who does not attend Rochester High School that wishes to attend a dance at RHS must enter with and attend with a current Rochester High School student.

Persons who are permitted to be a guest at a Rochester High School dance:

Current students attending Adams, Stoney Creek, ACE, or the IA.
Persons who are current high school students attending from a high school outside of Rochester Community Schools.
Persons who do not attend a high school in RCS and are 20 years old or under.

Persons who are not permitted to attend dances at RHS:

Middle school students (6-8 grade) are not permitted to attend.
Persons 21 years of age and older are not permitted to attend.

Dance tickets may only be purchased by current Rochester High School students.

Rochester High School reserves the right to deny entry to any person as warranted.

By entering your names below and submitting this form, you are agreeing to the following:

I hereby give my child permission to attend a dance at Rochester High School with a guest and acknowledge that the following conditions apply:

I understand that students will not be admitted to the dance after one hour from the start of the dance.
I understand that my child is obligated to observe all of the policies for Rochester High School, and failure to comply with those policies may result in my child and their Rochester High School host student being removed from the dance.
I understand that my child's guest must bring a picture ID (driver's license or school ID) and enter the dance with their host student.
I understand that as the host student, my child may be held responsible for the actions of their guest.
I understand that any issues involving substance abuse will be handled by police personnel in attendance at the dance.
I understand that students who are removed from the dance before its completion for a Code of Conduct violation will NOT receive a refund.
