  • 2.Drop-off: Parents/Guardians of participants in grades 3-6 must escort their children to class and leave them in the care of the instructor. Middle schoolers and high schoolers in grades 7th-10th may walk with written permission from parents.

    3. Pick-up: Parents/guardians of participants under the driving age must come to the class location to pick up their children. Parents/guardians of participants in grades 3rd-6th must sign their children out. Please remember to bring a form of identification. For the safety of participants, they will not be allowed to leave the school unattended to meet you outside in the parking lot or walk home. If you plan to carpool, list all people authorized to pick up your child on the emergency form. For special situations where you need to designate someone not on the form to pick up your child, either send in a written note or notify the instructor by email. Participants will not be released to anyone not listed on this form or with written permission from you. You are expected to pick up your child on time, or the late pick-up fee will apply (See LATE PICK UP for details Participants able to drive themselves must have permission granted from the checkbox on this form. Federation of Galaxy Explorers (FOGE) assumes no responsibility for participants after they leave.

    4. Returned Checks: Our bank charges fees for returned checks, so we must pass those fees along to the parents, therefore a service fee of $35.00 (subject to change if the bank fees change) for all returned checks. The participant may not return to class until payment is satisfied.

    5. Participant Conduct: This program runs under the umbrella, and with the permission, of the governing school or corporation. All participants are expected to conduct themselves respectfully and cooperatively and abide by the rules of Federation of Galaxy Explorers and the sponsoring organizations. The instructor will contact parents if their child fails to cooperate or behaves in a disruptive manner. Further instances of such behavior after the first notification may result in the participant's removal from the activity without a refund. 

    6. Late Pick-up: Parents must pick up Participants on time. The school or corporation has no legal responsibility for children enrolled in spring/summer camps/activities, mission teams, and other programs run by Federation of Galaxy Explorers (FOGE The instructors are responsible for Participants only during the designated FOGE program time. Furthermore, FOGE and its volunteer representatives are not legally liable or responsible for your children. To discourage late pick-up and use of FOGE as additional childcare, for which it is not intended, FOGE charges a late pick-up fee of $1.00 per minute beginning ten minutes after the stated end time of the class. Payment must be made payable to FOGE and given to the instructor at the time of pick-up. Participants cannot return to the program until payment is recieved, and if the late pick-up occurs during the last day, the Participant will not be allowed to enroll for any other program with FOGE. Any further late pick-ups will result in your child's removel from the program withour a refund. 

     This form is retained by the Summer Camp Director and FOGE Headquarters.

  • 1. All registration forms and fees must be submitted for a child to be registered. 2. The fee for the 2021-2022 Summer Camps: 

    $250 Moon Base One, Grades 3-4, July 26-30

    $250 Mars Base, Grades 5-6, July 18-23

    $300 Robotics, Grades 7-10, July 11-15

     

    3. Please submit a separate registration for each child.

    4.Registrations are non-refundable.

    All sections must be completed.

  • List two emergency contacts with phone numbers who could be contacted in an emergency and are authorized to pick up your child:

  • NOTE: In the FOGE programs, you agree to the following:

    "In the event that I cannot be reached to make arrangements for emergency medical attention, I authorize the program coordinator or other adult volunteers or staff present to seek immediate medical care at any facility that this person deems suitable. I further give my consent for any and all emergency medical treatment when the child/children are in this individual' care."

    Consent approval is implied by signature on the RELEASE.

  • Federation of Galaxy Explorers Participant Waiver & Release ("Release")

    Please read this document carefully before signing. Signature on this Waiver and Release is required for all FOGE Sponsored Activities. The Waiver is effective and will remain on file for the entire school year.

    Whereas the Federation of Galaxy Explorers (FOGE), as a service to its members and participants, provides various before and after school activities and other special programs for participants in 3rd-12th grades:

    This is to certify that the undersigned, as parent/guardian with legal responsibility for the child named below, a minor under the age of 18 (the Participant "), hereby consents to Participant's involvement in the 2021-2022 programs of the FOGE Mission Teams, special events, races, competitions, tournaments, field trips, and camps (collectively, the " Programs ") between August 1, 2021 and August 30, 2022 (the " School Year ") at various locations. In consideration for being allowed to participate in the Programs:

    1. The undersigned represents and warrants that they have the requisite legal authority and capacity to execute this Release on behalf of Participant.

    2. On behalf of Participant, the undersigned hereby knowingly and freely assumes all known and unknown risks, even if arising from the negligence of the Releasees or others, and assumes full responsibility for Participant's participation.

    3. On behalf of Participant, the undersigned hereby represents that the Participant has no medical or other conditions that would create any risk to the Participant in performing the tasks relating to the Programs.

    4. The undersigned hereby acknowledges and understands that the Participant must obey all instructions from Programs staff and volunteers regarding participation. For online programs, the undersigned also recognizes that acceptance of all software Terms of Use and Conditions is a prerequisite to enrollment by Participant and that the Releasees make no representations beyond these services.

    5. The undersigned, as parent/guardian with legal responsibility for the Participant, does hereby release, indemnify, and hold harmless the FOGE, including its officers, agents, members, volunteers and any person or persons in charge of running the Programs, (the Programs director and coordinators), sponsoring corporations and their employees, officers, agents, and volunteers, the Programs Vendors, their respective affiliates, successors and assigns and the officers, directors, employees, and agents of such parties and their affiliates, other participants, sponsoring agencies, sponsors, and advertisers, (the Releases ") from any and all claims, demands, suits, costs (including attorneys' fees and litigation costs) and charges, in connection with or arising out of the provided Programs, with respect to any and all injury, disability, death or loss or damage to person or property incident to Participant's involvement or participation in the programs, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law. I understand this Release includes any claims based on negligence, action, or inaction of the FOGE and the program director, coordinators, staff, and volunteers.

I further give my consent for any and all emergency medical treatment when the child/children are in this individual's care.

    7. We often take pictures of the children during the activities. These photos showcase the activities of participants within the program, what we are doing with the children, and what fun they are having. We do not release the names or addresses of the children to avoid identification. If you choose to opt-out of your child being photographed, please check off the opt-out statement below. Otherwise, FOGE assumes permission to be able to Photograph.

    This Release remains in effect for all Program participation by the Participant for the duration of the 2021-2022 program year.
