Informed Consent and Acknowledgement
I hereby give my approval for my child(ren)'s participation in this Soccer Camp with Motive8 Sports Coaching.
My child(ren) will wear appropriate clothing and other protection such as a hat or sunscreen to suit the weather conditions.
I have completed the medical details above and consent that in an emergency any medical treatment necessary can be administered to my child(ren), which would include anaesthetics.
I understand that while the coach(s) will take every reasonable precaution to ensure that accidents do not happen, they cannot be held responsible for any injury suffered to my child(ren).
Motive8 Sports Coaching accepts no liability for loss or damage to property.
A full refund will be given if you wish to cancel your booking up until 7 days before the event commences.
Photos/videos may be taken occasionally and used to promote Motive8 Sports Soccer Camps.
By signing this form I give Motive8 Sports Coaching permission to store my child(ren)'s and my confidential information.
