Equine Transfer Form
This form is for ownership transfers from Heart of Phoenix Equine Rescue to other equine rescue organizations. Heart of Phoenix Equine Rescue (HOP) agrees to surrender, transfer and release legal ownership of below described equine (s), to the party listed below. It is understood that the party assuming ownership will hold HOP, INC and all HOP’s officers, directors, employees, and volunteers harmless from any claims of damage, injury, or acts of negligence arising from this transfer. The transferring organization makes no assurances as to the current condition, illness, behavior, future quality of life or conditions that may arise in regards to the transferred equine(s). Heart of Phoenix requests the person or organization assuming ownership provide quality care of the equine, not participate in breeding or sell the equine(s) at auction or cause the equine(s) to be put into danger. This form is to be filled out by a HOP leadership representative.
