By signing this form, you consent to hold Heart of Phoenix Equine Rescue and all of the officers, board members and volunteers blameless and faultless when hauling horses, livestock or materials for the organization (HOP), including but not limited to: Personal injuries, personal truck/trailer or other equipment damage or property damage of any type. Further, if volunteering to haul for Heart of Phoenix, any damages to your personal truck/trailer or equipment, you assume responsibility for all costs, including attorney fees, replacement costs and damage repair. You also agree to use our recommended practices for humane loading of horses. Type "I AGREE"
