Thank you for your interest in my work, please ensure that you are reading through this form carefully and have read my Artist Requirements/Notes at the bottom of my form before inquiring so that you are aware of my process and how to prepare yourself for your appointment. This form is a pre-consultation to ensure I am the artist for you and your piece.

  • Flat Rate sessions: All of my tattoos are priced BY THE PIECE with a starting price of $200. After your form is submitted I will be in contact to discuss your design, pricing, (& amount of sessions needed) based on your references and budget.

  • Non-Refundable $200 Retainer Required For ALL appointments.

    This retainer is to hold a specific date/time and design time. All retainers go towards the total cost of the tattoo.

  • ARTIST REQUIRMENTS:

    I will be as thorough as I can be while discussing your piece with you. I am an artist that focuses heavily on the finer details of a piece. I note this to emphasize, any large concept piece WILL take multiple sessions. You will be informed ahead of time if that is the case. With that in mind I am not the artist for everyone , which is okay.

     

    I do not do cover up work.

    I do not re-work/finish other artists work.

     

    NOTES FOR ALL SESSIONS:

    *All Payment information will be discussed during consultation*

    • I take payment in form of Cash, or Venmo. 
      ***NOW ACCEPTING CREDIT CARDS - a 3% surcharge on card transactions.***
    • I communicate through email, therefore, please make sure you are checking your email the day of your appointment date/time just in case of any questions/changes prior to you coming in for your session.

    CANCELLATION POLICY:

    If your appointment needs to be rescheduled, I require a 24 hour minimum notice. The retainer will be rolled over one (1) time. If the appt needs to be rescheduled a second (2nd) time, a new retainer will be required to reschedule your session.

    If your session is canceled within less than 24hr notice, prior to the scheduled appt. I will require a new retainer to reschedule.
