CHARGES

Daycare pricing includes daily house keeping, group playtimes, and providing lunch / dinner per your request. (Please bring your pet's food. All other services requested will be changed as a la carte fees)

Dogs must be picked up within the daycare hours, which are Monday through Friday by 6 PM. If your dog does not get picked up by closing time (6 PM), they will be automatically placed into boarding and will receive boarding charges.

Hours: Monday-Friday 7 AM - 6 PM

Treatment Waiver

For the wellfare of all our guests, the following statements convey our Daycare's practices as approved by the veterinarians and management at Thrive Pet Healthcare at Natick:

We will attempt to contact you or the emergency contact listed on this document prior to performing any diagnostic tests or services deemed necessary to prescribe an appropriate treatment plan for a seriously ill or injured daycare attendee. Owners are responsible for all expenses associated with treatment services.

Dogs past 7 months needs to be neutered/spayed to be eligible for daycare and group play services.
I give you my permission to treat my pet at Thrive Pet Healthcare at Natick in the event of an emergency or if my pet gets injuredIn a non-emergency situation, please DO NOT TREAT my pet.
I give you my permission to treat me pet at Thrive Pet Healthcare at Natick and transport to emergency treatment facility (VEG of Newton) if the care surpasses what Thrive Pet Healthcare at Natick can giveI do NOT wish to have my pet referred to an emergency clinic in an emergency situation

Waiver

I hereby waive and release Thrive Pet Healthcare at Natick, its employees, directors, owners, representatives, and agents from all liability which my dog may suffer, including specifically but without limitation, any injury or damage whatsoever arising from my dog's attendance and participation at Thrive Pet Heatlhcare at Natick, Four Paws Doggie Day Care.

I recognize that the health of my dog is the owners responsibility. If the dog gets injured while at the Four Paws Doggie Day Care, we will offer a complimentary exam at OUR hospital. However, any services or medications that occur from this exam are NOT covered by the business and will be charged to the client's account.

I represent that all required vaccinations are up to date. I will also continue to ensure that the required vaccinations will be kept up to date for as long as my dog attends Thrive Pet Healthcare at Natick Four Paws Doggie Daycare. I will provide proof of all booster vaccinations to Thrive Pet Healthcare at Natick. If my dog enters doggie daycare with vaccinations that are out of date, I grant Thrive Pet Healthcare at Natick the right to provide such treatments to ensure my dog is up to date while here. I understand that I will be held financially responsible for all associated costs. I further understand and agree that in admitting my dog, Thrive Pet Healthcare at Natick has relied on my representation that my dog is in good health and has not harmed or shown aggression or threatening behavior towards any person or any other dog. I understand and agree that Thrive Pet Healthcare at Natick and their caregivers will NOT be liable for any problem that might develop with my dog, including, but not limited to: sickness, disease, injury, escape, and death.
