Healthcare & Conduct
In granting permission, I, Parent's name* understand that although band participation is a school-sponsored activity, and although there will be a teacher, staff, and parent chaperones present, and although they represent the Livonia Public Schools district, that neither the teacher, staff, parents, nor the School District are liable in case of an accident or injury that occurs during band activities.

In case of medical emergency, I understand that every effort will be made to contact the parents or guardians of the participant. In the event that I cannot be reached, I hereby give permission to the physician selected by the sponsor to hospitalize, secure proper treatment for, and to order injection, anesthesia, or surgery for my child as named above.

I further understand that my son/daughter will be expected to follow all rules set forth for all band activities. I promise that my son/daughter will not use, possess, or distribute any prohibited or illegal substances; including vape supplies, drugs, cigarettes, alcohol, or firearms, while participating in band activities.

Should student's name* fail to comply with these rules, I will come immediately when I am called and return him/her to my home, or I will arrange for that to happen as soon as possible. In agreement with the above, I therefore allow my son/daughter to participate in all Marching Band activities.
Travel Permission
Student's name* has my permission to travel by bus with the Marching Band for the 2022-2023 school year. I understand that my child is expected to follow the rules and regulations of student conduct on the bus that are based upon state safety regulations and are outlined in the Franklin High School Parent and Student Handbook. Although all due precaution will be taken to avoid accidents, it is agreed that in such a case, the school and/or teacher will not be held responsible.

Additionally, my child is allowed to ride with a Livonia Public Schools-approved parent/chaperone should buses not be utilized.

Commitment & Conflict of Interest

The following policies are in place to create an environment that will allow the Franklin Marching Band to reach the highest marching and musical potential. Please check each policy and sign below to indicate agreement.
Attendance at Band Camp is mandatory. We learn our entire field show at Camp. If anyone is missing for even a portion of Camp, it will seriously impact the progress of the entire show.Most Fall sports conflict with marching band. If you intend to participate in a Fall sport, you must discuss a shared schedule with Mrs. Kuchta and your coach prior to the start of the season.Students MAY take a 7th hour class and participate in marching band, please notify Mrs. Kuchta so that accommodations can be made.Attendance at all FHS home football games, Saturday Clinics/Competitions, and after school practices is mandatory. You are irreplaceable!Students must arrange their work schedules around marching band practices and events. Communicate with employers well in advance to avoid conflicts.Students (except Color Guard) must be enrolled in a band class or orchestra class to participate in Marching Band. Students should utilize the 7th hour gym, health or online learning options if necessary.Unavoidable schedule conflicts must be communicated in writing (or email) by a parent as soon as they are known. Emergencies will be addressed on an individual basis.Excessive absences and/or poor conduct may result in exclusion from a single performance (benching), suspension, or expulsion from the band.Participation fees for the 2022-2023 season are $150 (full amount due August 1st).
