Why am I signing consent?
Oxford University Hospitals NHSTrust has adopted a policy in line with the Data Protection Act which gives youthe right to control the future use of the photographs taken of you. As such we need your written consent before we take your photograph.
How and where will my photographs be used:
The use of photography plays an essential role in the promotion of Oxford University Hospitals NHS Foundation Trust,the University of Oxford and the NHS; as such your photographs may appear in allforms of media including web pages, social media, promotional literature, patientinformation etc. in relation to these organisations.
General Consent to OUH NHS Foundation Trust, University of Oxford and NHS Use
It has been explained to me and I understand that the photographs taken of me will be made available through Oxford Medical Illustration for the purpose of promoting the principles and practices of Oxford University Hospitals NHS Foundation Trust, the University of Oxford and the National Health Service, in a variety of media as detailed above.
I understand that no fee is payable to me in relation to these photographs.
I understand that although I may request their withdrawal from the library by contacting OMI, the images may already be in use and that an immediate withdrawal may not be possible.
