  • Hello, Doctoral Student-

    In order to complete the Admission to Candidacy form, please provide your contact information as well as the name and email address for your chair and committee members.

    Your chair and committee members will automatically receive an email with instructions for them to sign the form.

    Once your committee has signed the form, it will progress to the Program Director and Dean for additional signatures.

    Once complete, the system will email you and the Program Director a final PDF copy with all signatures for your records.

  • Ph.D. in Leadership Studies Program

     The Advisor has confirmed that the applicant meets the following requirements:

     * Has successfully completed the Leadership Studies core curriculum with their cohort

     * Has a minimum GPA of 3.0

     * Has completed LD 810 and 811 and is currently enrolled in, or has completed
        LD 901, 902, 903, or 904

     * Has no grades of incomplete appearing on transcript
