Some characters in this show eat candy, chocolate, or lollipops during the performance. Please list any food or dietary restrictions, concerns or things we should be aware of.
1. Please list any experience you have had in the performing arts on the back of this form.

2. Rehearsals are Mandatory, missed rehearsals may result in removal from scenes or performances.

3. Please mark all conflicts on the rehearsal calendar and turn in prior to leaving the audition. We request that actors miss no more than 3 rehearsals total during the production's run.

4. Every actor must be registered by Thursday, September 4th or Friday, September 5th at 4pm. Tuition covers the cost of the director's salary, scripts, licenses and royalties, sets, props, etc.

There are no refunds or credits on tuition or fees.

5. Every actor will be required to provide shoes; tights or socks based on his/her costume needs. Costumes will be chosen at the discretion of the Director and Costumer. All comments and concerns must be brought to the production staff and not to any volunteer personnel.

6. Every enrolled student will be cast, unless otherwise specified, for an individual part, part of the ensemble, may be double cast in a role or have more than one part depending on the number of cast members.
As of March 12, 2022, California Arts Academy is no longer requiring the use of face coverings, masks or shields inside our facilities. Masks will be provided upon request. Restrictions subject to change with developing conditions.

I understand that the novel coronavirus presents risks outside of California Arts Academy's control and by signing this form, I voluntarily choose to assume all responsibility for any injury, illness, or other conditions related to COVID-19 and release California Arts Academy of any liability related to COVID-19.

Additional Information for the Director to know?
The director will choose performance dates for members who are double cast. You may not request performance dates. If you can't make the performance dates assigned to you, you will forfeit your assigned performance dates to your double.

If you arrive late to a scheduled call time and did not notify California Arts Academy, you will forfeit that day's performance.

I have read the above mandatory dates and understand that conflicts with any of these dates may result in my child being ineligible for a lead part and/or performances. Conflicts given later than the signing of this contract will result in loss of a lead part and/or loss of performances.
