In applying to serve as an Elevate Youth volunteer, I agree that I:
1. Am 18 years of age or older.
2. Will submit a background check through the Department of Justice.

As an Elevate Youth Volunteer I will:
1. Represent Elevate Youth with professionalism, dignity, and be responsible for conducting myself with courtesy & appropriate behavior
2. Follow through and complete accepted tasks.
3. Dress and act in an appropriate manner at all times, and be a positive role model.
4. Display respect and courtesy for Elevate Youth employees, other volunteers, program participants, and property.
5. Provide a safe environment by not harming youth or adults in any way, whether through discrimination, sexual harassment, physical
force, verbal or mental abuse, neglect, or other harmful actions.
6. Respect the privacy of persons served by the organization and hold in confidence sensitive, private, and personal information. (Reports of
child abuse or neglect will be handled as per Massachusetts State Law and Elevate Youth policy.)
7. Keep Elevate Youth staff informed of progress, concerns and problems within the program (s) in which I participate.
8. Work cooperatively as a team member with Elevate Youth employees and other Elevate Youth volunteers.
9. Respect and follow Elevate Youth policies and program expectations.
10. Keep personal opinions and actions separate from those made as a representative of this organization

As an Elevate Youth volunteer I will not:
1. Use vulgar or inappropriate language.
2. Solicit gratuities, gifts, or bequests for personal or professional benefit.
3. Use or be under the influence of illegal drugs.
4. Consume or be under the influence of alcohol or consume tobacco at youth events.
5. Discriminate on the basis of race, color, religion, sex, age, national origin, marital status, disability or sexual orientation.

In consideration of the services of Elevate Youth, their agents, owners, officers, volunteers, participants, employees, and all other persons or entities acting in any capacity on their behalf (hereinafter collectively referred to as "EY"), I hereby agree to release, indemnify, and discharge EY, on behalf of myself, my spouse, my children, my parents, my heirs, assigns, personal representative and estate as follows:

I acknowledge that my participation in hiking, walking, biking, rock climbing, skiing and/or snowboarding, paddle boarding, swimming, surfing, snorkeling and other adventure based activities entails known and unanticipated risks that could result in physical or emotional injury, paralysis, death, or damage to myself, to property, or to third parties. I understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of the activity.

The risks include, among other things: Slips and falls; accidents involving other bicycles or vehicles; collision with fixed or movable objects; injuries or accidents involving contact with the bicycle; falls from the bicycle; the negligence of other operators of motor vehicles or myself; cuts, bruises, abrasions, and concussions; the use of climbing ropes and equipment; rope burns; weather conditions; falling objects; water hazards; falling into the water, and accidental drowning; exhaustion; exposure to temperature and weather extremes which could cause hypothermia, hyperthermia (heat related illnesses), heat exhaustion, sunburn, dehydration; and exposure to potentially dangerous wild animals, insect bites, aggressive and/or poisonous marine life and hazardous plant life; equipment failure; and improper lifting or carrying; hidden obstacles by snow including crevasses, ice and snow cornices, tree wells, tree stumps, creeks rocks and boulders, below the snow surface; loss or damage to equipment being used; being lost or separated from their guides or companions by traveling in forested areas, rugged terrain, or bad weather; exposure to altitude and cold including hypothermia, frostbite, acute mountain sickness, exhaustion, cerebral and pulmonary edema; my own physical condition, and the physical exertion associated with this activity; the condition of roads, terrain, or highways and accidents connected with their use.

Furthermore, EY employees have difficult jobs to perform. They seek safety, but they are not infallible. They might be unaware of a participant's fitness or abilities. They might misjudge the weather or other environmental conditions. They may give incomplete warnings or instructions, and the equipment being used might malfunction.

1. I expressly agree and promise to accept and assume all of the risks existing in this activity. My participation in this activity is purely voluntary, and I elect to participate in spite of the risks.
2. I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless EY from any and all claims, demands, or causes of action, which are in any way connected with my participation in this activity or my use of EY 's equipment or facilities, including any such claims which allege negligent acts or omissions of EY.
3. Should EY or anyone acting on their behalf, be required to incur attorney's fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.
4. I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating, or else I agree to bear the costs of such injury or damage myself. I further certify that I am willing to assume the risk of any medical or physical condition I may have.

By signing this document, I acknowledge that if anyone is hurt or property is damaged during my participation in this activity, I may be found by a court of law to have waived my right to maintain a lawsuit against EY on the basis of any claim from which I have released them herein.

I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.

• I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
• I further acknowledge that Elevate Youth has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
• I further acknowledge that Elevate Youth can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Elevate Youth staff, and other Elevate Youth clients and their families.
• I voluntarily seek services provided by Elevate Youth and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending Elevate Youth Programming.
• I attest that:
  
  • I am fully vaccinated for COVID-19. 
  • I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
  • I have not traveled internationally within the last 14 days.
  • I have not traveled to a highly impacted area within the United States of America in the last 14 days.
  • I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
  • I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
  • I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
  • I hereby release and agree to hold Elevate Youth harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the organization, or that may otherwise arise in any way in connection with any services received from Elevate Youth. I understand that this release discharges Elevate Youth from any liability or claim that I, my heirs, or any personal representatives may have against the organization with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Elevate Youth. This liability waiver and release extends to the organization together with all owners, partners, and employees.
