    This form should only be completed if you have a confirmed offer of a place at our school. The information will be transferred to our Student Information Management System.

  • Parental Responsibility


    Please list the names of all those who have parental responsibility for the child. Normally the first two contacts will be the parent(s) with responsibility i.e. birth parents (step parents will normally only have parental responsibility if they have legally adopted the child).
     
    From time to time there may be occasions when correspondence will be sent separately to both parents. This may result in all those with parental responsibility seeing the home address and email address of each other. 

    If any of those with parental responsibility have any specific legal orders in place, then please specify below and you MUST provide us with a copy of the Order before your child starts at the school in September.

  • Cycle Permit Agreement


    I have read and understood the following terms and conditions, and agree to follow them.Students should be competent cyclists and obey the Highway Code.

    • Cycles should be in a roadworthy condition.
    • Cyclists must dismount at the school gates.
    • Cycles must be walked to and from the school gates.
    • No students are permitted near the cycle storage area during the school day.
    • Cyclists must not loiter in the cycle storage area.
    • Cyclists must close the gate securely when they leave.
    • Cyclists must not move, touch or interfere with any cycle not belonging to them.
    • Cyclists are strongly recommended to wear a safety helmet.


  • Ethnic Based Statistics

    Completion of this part of the form is voluntary but please help us to improve our Education Service by providing the information you feel is correct for your child in each of the following sections.

    Ethnicity is a category of people who identify with each other based on similarities, such as common ancestral, language, social, culture or national experiences. Examples may be White – British, White – Other, Black – African, etc.
     
    First language (also native language, father tongue/mother tongue, arterial language) is the language or are the languages a person has been exposed to during early development and continues to be exposed to in the home or community.  Examples may be English, Spanish, Polish, etc.
     
    National identity is one’s identity or sense of belonging to one state or to one nation. Examples may be British, Irish, etc.
