All Fidelity Pups Clients using Boarding, Daycare, and Transport must complete and sign the Waiver.
I understand that I am solely responsible for any harm caused by my dog(s) while my dog(s) is/are attending daycare/boarding at Fidelity Pups.
I also understand and agree that in admitting my dog(s), Fidelity Pups has relied upon my representation that my dog(s) is/are in good health and have not harmed or shown aggressive or threatening behavior towards any person or any other dog.
I authorize use of my dog’s visual images and statements in Fidelity Pups, LLC newsletters, posters, and other materials.
I understand that there are many benefits to sending my dog to a dog activity center, and I understand that the benefits outweigh the risks of illness and injury.
I further understand that due to the way that dogs interact with one another, cuts, scratches, nips, bites, and injury can occur even though the dogs are carefully supervised at all times.
I further understand that due to the way that dogs interact with one another - drinking out of the same water bowl, smelling and licking eachother etc, illness and disease can be spread easily - giardia, kennel cough, flu, colds etc. (Even if vaccinated).
While my dog(s) is/are in the care and custody of Fidelity Pups, if I am unreachable in the event of an emergency, I hereby authorize Fidelity Pups, its agents, and/or representatives to seek immediate veterinary care for my dog.
I understand that all costs in connection with, veterinary, medical or other treatment shall be my responsibility.
I hereby release and agree to save and hold harmless, Fidelity Pups, it’s directors, officers, shareholders, employees, assistants, members and agents from any and all liability, claims, suits, actions, loss, injury or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I or my dog(s) may sustain or which may be caused in any way by my dog(s). I specifically, without limitation, agree to fully indemnify Fidelity Pups for any and all such liability, claims, suits, actions, losses, injury or damage.
By signing this document, I acknowledge that if my dog is or I am hurt or my property is damaged during my dog’s participation activities at Fidelity Pups. I may be found by a court of law to have waived my right to maintain a lawsuit against Fidelity Pups on the basis of any claim from which I have released them herein. I have had sufficient opportunity to read and fully understand this entire document and I agree to be legally bound by its terms. I have reviewed and submitted this liability waiver to Fidelity Pups. By signing my full name, I certify that all data I have provided and agreed to on this form is correct and truthful to the best of my knowledge
Although we carefully screen all applicants, occasionally we discover that this is not an appropriate environment for every dog. Fidelity Pups reserves the right to permanently remove a dog from daycare at any time. In addition, Fidelity Pups reserves the right to disallow certain breeds to participate in the daycare.
*Fidelity Pups requires a two-day minimum per week (for daycare) all year long.
*Fidelity Pups is a week-to-week commitment. It is $40 per day with a two-day per week set schedule no matter how many days you come per week. It is a set schedule that is saved for you dog each week. You are purchasing your specific schedule each week. Your pup must be at your home for the pickup. If your pup is not home/ready for The Fidelity Pups Bus! there are no refunds/make-up days.
*The spot cannot be used by another dog. If you cancel there are no refunds,make up days or credits for other services. Daycare is prepaid each week. If it is not paid before the next week, service will be suspsended and the spot will be given to a dog on our waitlist. Days cannot be cancelled in the week and then moved to another week.
*Holiday weeks you are NOT charged for days we are closed. A 2 day notice is REQUIRED to cancel daycare.
*Transport can be closed due to poor weather conditions. If the roads are unsafe for the drivers & dogs, We will close transportation!
*We do not refund transport days if we have to close due to unsafe driving conditions*
