Health Certification and Medical Release
I agree that my health is adequate for physical activities associated with playing racket or paddle sports, physical athletic training and any other activities undertaken while associated with John Hebert Tennis ™, John Hebert Pickleball ™, Sweet’s Property Management™, Hebert Engineering Consultants LLC and any participating facility. I hereby consent to allow emergency first aid and other medical procedures, or hospital services that may be rendered by or at accredited hospitals, by appointed physicians, which at the time of injury or illness seem reasonably advisable. I also recognize that there is a possibility of exposure pathogens during these activities. I accept that participation in sports instruction occassionally requires physical contact with other players and coaches.
Liability Waiver and Indemnity Agreement
Acceptance of my entry and all use of the participating facility and equipment is without responsibility of any kind by John Hebert, John Hebert Tennis ™, John Hebert Pickleball ™, Sweet’s Property Management™, Hebert Engineering Consultants LLC and its coaches and staff along with any participating facility its associations, their officers, organizations, committees, management and owners. In consideration of the acceptance to participate, I do hereby for and on behalf of myself and my heirs and legal representatives release and forever discharge John Hebert, John Hebert Tennis ™, John Hebert Pickleball ™, Sweet’s Property Management™, Hebert Engineering Consultants LLC and its coaches and staff along with any participating facility, its associations, their officers, organizations, committees, management, owners, representatives and their successors and assigns, of and from any and all claims, demands, and injuries, however arising, whether caused by the negligent or intentional acts of any personnel or representatives in these groups, representatives of other sponsoring entities, or by third parties, which injuries may be in any way related to my activities on facility property for any reason, during class sessions, any activities on court and any period traveling to or from activities, and all such claims are hereby waived and released, and I covenant not to sue therefore. The individual, by signing below, does hereby agree to indemnify and hold harmless John Hebert, John Hebert Tennis ™, John Hebert Pickleball ™, Sweet’s Property Management™, Hebert Engineering Consultants LLC and its coaches and staff along with any participating facility its associations, their officers, organizations, committees, management and owners from any liability which they may incur, howsoever arising and whether caused by the negligent or intentional acts of the coaches, staff, representatives or other players.
