OWNER 'S CONSENT TO TREAT
I declare I am/am acting on behalf of the legal owner of the above-named animal and that all information provided is correct to the best of my knowledge. I give consent for this animal to be assessed and treated accordingly by Lucie Edwards (Lucie the Vet Physio).
VETERINARY HISTORY AND VETERINARY PERMISSION
I accept full responsibility for divulging facts that may be relevant to treatment, particularly regarding any changes in my animal’s health and behaviour.
Veterinary consent will not be required if the animal is a healthy animal with no underlying injury, disease or pathology, providing the animal is registered with a Veterinary practice (RCVS Standards Committee (Dec 2020)). Veterinary permission will be sought if the animal is booked in with a diagnosed or undiagnosed musculoskeletal pathology (injury or musculoskeletal disease).
If treatment is booked as maintenance, but on assessment there are concerns relating to underlying injury, disease, or pathology, treatment will not be continued and you will be advised to contact your animal's veterinary surgeon. Please understand there may be an assessment fee for the time taken and assessment provided if treatment was not possible.
DATA PROTECTION
In line with the General Data Protection Regulation, I agree to the collection and storage of my contact details by Lucie Edwards which will be kept for contact reasons only and not shared with anyone that is not part of the animal’s multidisciplinary team, e.g. veterinary surgeon. Contact details and patient notes will be stored electronically on a password-protected device. I understand I can revoke this consent at any time with a written notice.
I understand that a report detailing the assessment and treatment of my animal will be sent to the veterinary surgeon for their knowledge and veterinary record. I understand my veterinary surgeon will be contacted if there are any concerns about the animal’s musculoskeletal health at the time of examination. Additionally, if treatments are not progressing as successfully as expected by the therapist, my Veterinary Surgeon may be contacted by Lucie Edwards for discussion and their input.
SAFETY DURING CONSULTATIONS
All animals must be adequately restrained during assessment and treatment for the safety of all involved. The majority of patients receiving physiotherapy will be experiencing discomfort which can evoke behaviours not seen by the owners in everyday life. ALL horses and dogs must be held by the owner or other competent person during assessment and treatment.
Dogs must wear a well-fitting collar and lead.
Horses must wear a well-fitting headcollar/halter/bridle (as suitable) and leadrope.
Please disclose any knowledge of previous aggressive behaviour shown by your dog/horse to Lucie at the time of the appointment.
It is at the discretion of the physiotherapist, Lucie, if a dog is required to wear a muzzle for safe completion of assessment and treatment. If at any time Lucie feels that a part of the consultation is unsafe, it is Lucie's right to cease treatment. Full charges will still apply for the time and professional advice provided.
DEPOSITS AND CANCELLATIONS
For a new client's first appointment, a non-refundable 50% deposit is required 48 hours before the appointment.
Cancellation of any appointment within 48 hours of the appointment without mitigating circumstances will incur a charge of 50% of the booked appointment fee. Please provide details at the time of cancellation if you feel your circumstances warrant the fee to be waived; I try to be as fair and accommodating as possible.
PAYMENT
Full payment via BACS or cash is required before or at the time of treatment.
