Each guest of Unleashed Fitness shall be liable for any property damage and/or personal injury (caused by the guest, Guest’s Family, member, or any other person) at Unleashed Fitness or any activity or function operated, arranged, or sponsored by Unleashed Fitness. It shall be the obligation of the member to pay for any costs involved upon presentation of a statement thereof. All use of Unleashed Fitness Facilities, or participation in Unleashed Fitness activities operated, arranged, or sponsored by Unleashed Fitness either on or off Unleashed Fitness premises by the guest, guest’s Family or member shall be AT SUCH PERSON’S OWN RISK, and Unleashed Fitness shall not be liable for any injuries or damages to such person, or the property of such person, or be subject to any claim, demand, injury, or damages. The Guest individually, and on behalf of the Guests’ personal representative, heirs, administrators, assigns and successors does hereby expressly forever release and discharge Unleashed Fitness, its successors, and assigns, as well as its officers, agents and employees from all such claims, demands, actions, or causes of action.

Rules & Regulations **Make Sure to read before you initial**

Initial Here* Re-Rack all weights & put away all equipment in it's proper location after use. Failure to clean up after yourself will result in a gym ban.
Initial Here* Wipe off all equipment after use with gym wipes provided and throw away wipes in trash can. If you use chalk or baby powder, clean it up with the mop provided in the restroom!
NO LIQUID CHALK. We provide regular chalk!
Initial Here* Do not drop any bars loaded or unloaded. You are liable for any damages you cause to our equipment due to gross negligence.
Initial Here* Proper workout attire is required. Shoes and/or socks must be worn at all times. No bare feet. No dirty/muddy shoes or boots allowed. Shirts may be off when training on the barbell side of gym. Shirts must be worn if skin is in contact with equipment on the front half of gym.
Initial Here* No outside personal training. Only Unleashed Fitness employees/trainers are allowed to train clients in our facility.
Initial Here* Don't take anything that doesn't belong to you. If you're caught stealing from Unleashed Fitness or from another member, your access will be banned, & we'll contact local law enforcement.
Initial Here* Don't open the door for anyone!! Allowing non-members into Unleashed Fitness will get you banned from our facility.
