Please note that you must be at least 18 years old in order to sign a liability waiver. If you are not 18 years old, please ask your parent or legal guardian to complete this process.

I give my permission for my young person(s) to be transported to and from the stated activities sponsored by Paper Planes Ministries. I understand that my young person(s) will be transported to/from Paper Planes Activities. If for any reason my child decides to leave an event by walking home or finding another ride, I do not hold Paper Planes Ministries responsible for any injury to any person(s) or loss/damage of property. I consent for them to travel in the transportation provided and participate in the activites at the stated event. I do not hold Paper Planes Ministries responsible for any injury or loss/damage of property.

In addition, I consent to the following discplinary actions made by the staff and volunteers of Paper Planes Ministries:

1. Any acts of violence or incitement of violence by my child(s) will result in a suspension period determined by the staff where attending Paper Planes events will not be permissible.

2. If my child(s) leaves a Paper Planes event by another mode of transportation other than a Paper Planes vehicle that has not been consented by a staff member they will receive a suspension of a length of time determined by the staff.

3. My child(s) is aware that if any other disciplinary problems may occur while attending a Paper Planes event they will receive a "strike" which results in immediately being sent home.

4. My child(s) is aware that if they receive three "strikes" they will receive a suspension period determined by the staff where attending Paper Planes events will not be permissible.

5. My Child(s) is aware that Paper Planes Ministries, and any other partners in leadership, have the right to address any actions that may be disruptive to the learning environment in any way that maintains a safe environment for other children to learn.
