This form must be completed before volunteering in any capacity at a VAISD campus or activity.
Pursuant to Texas Education Code Section 22.083-22.086 Texas public school districts may obtain criminal history records on any volunteers which includes accompanying adults for field trips. Be informed and understand that this school district shall obtain criminal history record information. DBAA(LOCAL)
You may be disqualified from volunteering with this school district if you have been convicted of a felony or any offense involving moral turpitude or for not disclosing any conviction.
An applicant to the district may be disqualified if his or her criminal history report or other official record shows:
Conviction, probation (including deferred adjudication), or adjudication for the following offenses:
a. Any offense against a child
b. Any sexual offense
c. Any offense involving an act (or threat) of violence, or injury or death of a person
d. Any weapons related offense
e. Forgery, fraud, or alteration of an official document
f. Felony theft related offense
g. Misdemeanor theft related offenses within 5 years
h. Public intoxication or impaired driving (e.g. DWI/DUI) related offenses within ten years.
i. Any felony offense involving drugs or controlled substances.
j. Misdemeanor drug offenses within ten years of the date of application.
k. Burglary related offenses.
l. Stalking, kidnapping, or harassment related offenses.
m. An active warrant for arrest or charges pending on any criminal violation.
n. Any offense that the superintendent believes may compromise the safety, security, or well-being of students, staff, or facilities or which violates District policy.
o. An arrest for any of the offenses described above, where the conduct underlying the arrest indicates the applicant is unsuitable for volunteering/attending field trips.
p. An excessive number of arrests, regardless of type, where the circumstances or pattern of arrests indicates the applicant is unsuitable for volunteering/attending field trips.
q. Required registration as a sex offender.
This criminal history record search may include local, state and national law enforcement agency records and you hereby expressly release any law enforcement agency, record holder or individual from any and all liability for releasing any and all information these agencies or individuals may provide. If there is a need for further clarification of your identity, you agree to provide additional information including but not limited to, photographs and fingerprints.
If requested, you will be provided the name, address, and telephone number of the reporting agency as well as the nature and substance of the criminal record information and the source. The review and confirmation must be obtained from the providing agency.
