I have made a personal commitment to Jesus Christ. I have read and agree with the "Statement of Fundamental and Essential" Truths of Calvary Temple and will not teach/promote different doctrines. (see link above) I understand my services are voluntary. I have submitted the names of three references and acknowledge that the volunteer coordinator will contact them I understand that my omission of information or misleading information is grounds for rejection of this application and/or removal from any assigned position. I give permission for my personal data to be entered on the church database system and understand that only authorized personnel will have access to this information. I understand that the Leadership Team will hold all personal information confidential. I commit to continuous development of my personal relationship with Jesus Christ and to the work He has given me to do in His service. I will honour all rules, respect and refrain from sharing any confidential information that I am exposed to whil service, and will honour the agreed upon commitment to the best of my ability. I will support the ministry area where I am involved at Calvary Temple, its doctrines, principles and philosphy. I will cooperate on a team with others in this ministry and place the greater good of the whole group over my personal desires. I will make myself available to attend any scheduled training and/or orientation sessions and meetings, as I am able. Should my application be accepted, I agree to support the leadership of Calvary Temple and to restrain from unscriptural conduct in the performance of my services on behalf of this ministry. I understand that by signing this agreement I will be held accountable to what I have agreed to and that I will be subject to an ongoing evaluation process. I affirm to the best of my knowledge that the information on this application is correct.
