Dear Parents/Guardians,

I hope this letter finds you safe and healthy.

We would like to extend our sincere appreciation for parents' cooperation on taking the first steps to quarantine your child and informing us about your child's health status.

According to KKM's procedures:

1. Positive cases with incomplete or no vaccination have to undergo home isolation for 10 days.
Therefore, please submit your child's RTK test result to school on the 10th day and come back to school on the 11th day.

2. Close contacts with incomplete or no vaccination have to undergo home isolation for 7 days.
Therefore, please submit your child's RTK test result to school on the 7th day and come back to school on the 8th day.

Please be patient with us in our arrangement as we try our very best to practice preventive measures to ensure students' and teachers' safety.

To prevent the spread of COVID-19 in our community and reduce the risk to our students and staffs, parents and/or guardians are required to complete this Health Declaration form on behalf of your child/children before returning to school on the 11th day (positive cases) / on the 8th day (close contacts). Your participation is important to help us to take precautionary measures to protect your child/children and everyone in our school. Thank you for your time.
