Waiver and Release
I hereby waive, release, and discharge KA Basketball, its employees, and the camp location from any and all rights and claims resulting from injuries to my child or property which may be sustained in connection with his/her association with, or participation in, or arising out of his/her traveling to or from camp. I allow KA Basketball staff to approve any emergency treatment to my child if I can not be reached. We, the parents or guardians, adhere to the above waiver and release. I hereby grant the permission to KA Basketball to use any photo in any and all of its publications, including website entries, without payment or any other consideration.
