COLLEGE VISITATION FORM

This form must be submitted by a student's Parent or Guardian BEFORE a Central Columbia High School student participates in a college visit, college placement testing and scheduling of college classes.

Students/Parents/Guardians need to obtain some form of verification signed by a College representative as proof of attendance and submit to the High School secretary upon return to school.
