    You are hereby notified of the possible risks and dangers associated with the application of each tatoo. These risk and dangers include, but are not limited to, at least the following:

    I. the possibility of discomfort or pain;

    II. the permanence of the markings;

    III. the risk of infection; and

    IV. the possibility of allergic reaction to the pigments or other materials used

     

    NO PERSON MAY BE TATTOOED WHO APPEARS TO BE UNDER THE INFLUENCE OF ALCOHOL OR DRUGS

     

    Please note that you will be required to provide a valid I.D. on the day of your appointment (to verifiy proof of age) and a copy will be made (for records) prior to being serviced

  • Minors need the written consent of a parent or legal guardian to receive a tattoo. An artist may not tattoo a person younger than 18 years of age without meeting the requirements of legislature of the State of Hawai'i Chapter (S321), whose parent or guardian determines it to be in the best interest of the minor in written notice prior to application of a permanent tattoo.
