Thank you for your interest in attending 50th Anniversary Celebration (Eyes on New York + EyeBall) at the Edison Ballroom.

The State University of New York requires all students, including individuals attending continuing education programs, to be fully vaccinated against COVID-19 in order to participate in programs on campus. Further, to ensure the health and safety of our community, our campus strongly encourages vaccination. Accordingly, we are requiring that all persons participating in the 50th Anniversary Celebration event attest, at the time of registration, that they are either fully vaccinated, or they have or are eligible for an exemption from the COVID-19 vaccination requirement. Further, they acknowledge that the risk of contracting COVID-19 is a possibility even with enhanced health and safety protocols in place.

Proof of vaccination must be provided if requested upon arrival to the Edison Ballroom and/or the SUNY College of Optometry staff.

Vaccination exemptions issued by your institution, employer, medical provider, or other qualified entity will be honored, though exempted individuals will be required to obtain a PCR test within 72 hours of the event and provide testing results upon arrival.

COVID-19 Safety Requirements

Event participants will be required to comply with all College of Optometry COVID-19 safety protocols, as well as any protocols required by the Edison Ballroom, which may include among other things, the wearing of a mask except when actively eating or drinking and providing proof of vaccination or negative PCR test.

In addition, participants agree to self-monitor for signs and symptoms of COVID-19 (typical symptoms include fever, cough and shortness of breath), and agree that if they are exhibiting symptoms prior to the event, they will not enter the College or event venue. Should a participant develop COVID-19 symptoms while at the event, they should immediately leave the building and seek medical attention. If they test positive for COVID-19 either during the event, or within 7 days thereafter, they should notify eyesonnewyork@sunyopt.edu immediately.
