    In order to provide a safe and secure environment for our children and youth, we believe it is necessary to ask the following questions as part of our application process. All information will be kept in confidence by SBCC leadership and the Plan to Protect® team and will not be disclosed by the camp unless required by law.  Answering ‘yes’ to any of the following questions may not necessarily prevent you from working with the camp. Thank you in advance for your understanding.
  • Are there any circumstances involving your lifestyle or background that would call into question your ability to work with children and youth? (e.g. use of illegal substances, etc.)*
  • Have you ever been convicted or found guilty of a criminal offense for which a pardon has not been granted (excluding minor traffic violations)?*
  • Have you ever been expelled from or had your employment terminated by any organization or employer for assault, violence or impropriety against a child, youth or vulnerable person (e.g. senior citizen or person with disabilities)?*
  • Have you ever been investigated by a child welfare agency or any other organization for suspected child abuse?*
  • Have you ever been a defendant or respondent in a civil lawsuit or human rights complaint or other legal proceeding in which you were alleged to have abused or engaged in violence, harassment or other immoral or illegal behaviour or conduct involving children or youth?*

  • Confidentiality Statement

    I understand that I will obtain or have access to sensitive and confidential information in my role as a paid personnel with SBCC.  That confidential information may include, without restriction, personal information regarding paid personnel, volunteers, members, attendees, supporters or persons participating in the SBCC camp program including information relating to donations, personal or family matters, or obtained from background screening of prospective program personnel.  I agree to maintain strict confidentiality of all such confidential information and I will not disclose such information to anyone (including to paid personnel, volunteers, members, attendees, supporters, persons participating in the SBCC camp program, or my spouse or family members) except authorized representatives of SBCC who need to know such information or as required by law.  I understand and agree that confidentiality is very important in my role and critical to the effective functioning of camp.  If I become aware that any confidential information was improperly disclosed, I will immediately advise the Director of Camp Operations.

  • I hereby give SBCC permission to contact persons named by me above as references and contact persons at my current and former employers to gather information in order for SBCC to decide whether I am suitable for work with children, youth and vulnerable persons.

     I hereby give SBCC consent to verify the information provided by me in this paid personnel application form and to contact the references and current and former employers listed above and to obtain and verify any information from them (and any other persons that SBCC determines might be able to provide relevant information) that may be relevant to my application.

     I will provide SBCC with my criminal records check (current within the last 3 years), and I will sign and return the attached “Release of Information and Declaration of Intent” for such purpose.

     I further grant SBCC permission to perform an internet search on me and to review and consider any information found about me on the Internet.

     I understand that if SBCC approves my paid personnel application and later determines, in its discretion, at any time that I am not suitable for service at SBCC or for the position for which I am applying, SBCC may terminate my service or position for any reason without advance notice.

     If SBCC approves my application for a position, I will sign any documents that SBCC requires and will at all times cooperate fully with the program personnel of SBCC in the fulfillment of my duties and will keep all confidential information I encounter in my role as a SBCC employee, confidential.

     If at any time, I determine that for any reason I am unable to support or adhere to or follow the policies, procedures or doctrine of the camp, I will inform SBCC and will resign my position.

     I hereby acknowledge that, to the best of my knowledge, the information contained in this paid personnel application form is true and correct.

     Information received is confidential and is being gathered for the purposes of considering your application for work with SBCC and for determining what, if any camp programs, you may be suited for in future.

  • Confirmation BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
