As the sole person filling out this application, I understand that I am now the person that will do all communications for this transaction, and any others in the future. There will be NO 3rd party people involved in the adoption of this puppy, other than a SPOUSE. Do you understand your future puppy is to be seen by your own primary vet within the first 72 hours that your puppy goes home? This is required for our Guarantee to take affect.
