Informed Consent and Acknowledgement
I hereby give my approval for my child's participation in this coaching course.
My child will wear appropriate clothing and other protection such as a hat or sunscreen to suit the weather conditions.
I have completed the medical details above and consent that in an emergency any medical treatment necessary can be administered to my child, which would include anaesthetics.
I understand that while the coach(s) will take every reasonable precaution to ensure that accidents do not happen, they cannot be held responsible for any injury suffered to my child.
I understand that we can accept no liability for loss or damage to property.
A full refund will be given if you wish to cancel your booking up until 7 days before the event commences.
We strongly recommend that every child is brought to the pitch where our Coaches will be present at the start of each session. At the end of each session we also strongly recommend that every child is also collected by a parent/guardian again from our Coaches from the pitch.
Whilst we appreciate that some of the older children are of an age that they walk to and from school each day we do recommend that they are still dropped off and and collected from the pitch. If you do give consent for your child to leave on their own at the end we cannot be held responsible for your child safety when they leave the pitch.
Unless we get your written consent for your child to leave on their own at the end of the sessions in the below section (Notes or Questions) your child will have to remain with our Coaches until they are collected.
