• Parent/Student Agreement 2022-2023

    Carefully read through the following expectations and policies with your student(s) so everyone is aware and ready to have a successful year at BTM. You keep the first four pages for reference.

    BTM Expectations:

    Bridge Tutorial Ministries helps homeschooling families by providing:

    • middle and high school level classes using structured curriculum
      skilled tutors with knowledge of subject areas
    • an interactive classroom environment (with lecture, discussion, projects and instruction) for students two days a week
    • weekly assignments, due dates, and grades
    • a positive learning environment for students
    • an overall grade for the class prayer for BTM and encouragement towards strong, healthy relationships within each home and with Christ

    Parent Expectations:

    It is the responsibility of parents to:

    • be the primary teacher for their child for all state required courses
      purchase all texts and supplies required for their student's classes prior to the first day of class
    • check email from BTM Administration/Tutors multiple times/week and respond promptly
    • make sure their child is completing assignments to their best ability and according to deadlines
    • create an environment in the home that is conducive to learning and set up for the best possible success of your student
    • make sure your student(s) arrive to their classes on time and are fully prepared
    • ensure that your student is picked up on time, 2:30 sharp if student does not have a period 6 class
    • communicate information regularly with your Tutors and the Leadership Team in order to promote a positive learning experience
    • pay tutors according to the agreed schedule/ specific guidelines of the Financial Agreement
    • fully complete Designated Parent responsibilities at BTM, including arriving on time and staying for the allotted duty time until dismissed by a BTM administrative member
    • ensure that your student(s) is following all BTM policies, including adhering to the BTM Dress Code, Electronics, Student Center, Plagiarism, Late Work, Dismissal and Safety Policies
    • pray for your student(s) academic and social experiences at BTM
    • comply with BTM Leadership disciplinary/restorative measures asked of your student(s)/family
    • allow flexibility in your schedule in the event that your student loses Student Center privileges and your presence may be necessary for him/her to continue taking BTM classes
    • ensure your student is using the Student Center only for the appropriate number of periods according to the Student Center policy

  • Student Expectations:

    It is the responsibility of students to:

    • arrive on time to class and complete all assignments on time,
    • honor God and each other in everything, including verbal exchanges as well as written or electronic communications; do not use curse words or take God's name in vain; no vulgarity, harsh words, or suggestive sexual rhetoric is permitted in word, deed, pictures, videos or any other means
    • help to care for our building and surrounding areas, including cleaning up after oneself in the Student Center
    • be positive and respectful to oneself as well as others in speech and in physical contact, respecting others' personal space, this includes, but is not limited to: no frontal hugging, hand-holding, lap sitting, excessive bodily contact, kissing, or touching others without their consent while at BTM or anywhere on the premises
    • discourage bullying of any kind and report bullying or any other serious concerns to BTM tutors and/or administration
    • dress modestly and appropriately according to the stated Dress Code Policy
    • adhere to all BTM policies, including the BTM Dress Code, Electronics, Student Center, Plagiarism, Late Work and Safety Policies
    • display honesty and integrity in all things relating to BTM
    • stay in the vicinity of an adult while at BTM and on the host facility's property, including when outside and in the parking lot
    • not bring or use weapons of ANY sort at BTM
    • not bring alcohol, drugs, or any smoking device to BTM; not use any illegal substances or smoking devices at BTM, or arrive at BTM under the influence of any illegal substances; to report the use of drugs, alcohol, or any smoking device at BTM to tutors and/or administration
    • attend all registered classes while at BTM; do not skip class
    • return to class or Student Center immediately after restroom use
    • gather and keep all belongings with you prior to and during the last class period of the day

    Plagiarism Policy

    Plagiarism will not be tolerated and will result in dismissal from class. Acts of plagiarism can include, but are not limited to:

    • using words or ideas from a published source or the internet without proper permission
    • using the work of another student (e.g., copying another student's homework, composition, or project in entirety or in part)
    • using excessive editing suggestions of another student, teacher, or parent
    • using any words from a source verbatim without using quotations and citing the source
    • using someone else's ideas from a source and passing them off as your own, without using proper citations
    • changing words but copying the sentence structure of a source without giving credit
    • copying so many words or ideas from a source that it makes up the majority of your work, whether you give credit or not

    Late Work Policy

    Tutors will provide clear due dates and expect students to complete and turn in their work on those designated days. Work is due at the beginning of the class period, or per the tutor's instruction.

    Many times the work assigned prior to class time is necessary to prepare the student to participate in the class. Below are the expectations for completed assignments.

    • Students are to complete their work on the day it is assigned and to turn it in on the due date.
    • Parents are to review and approve their student's work before it is turned in to the tutor.
    • As students are to turn in their assignments to the tutor at the beginning of class (see above), students should not be working on homework assignments during class time unless approved by the tutor.
    • It is the tutor's discretion whether to accept late work. Their policy for accepting and grading late work will be explained in each class welcome letter and syllabus. The tutor may choose to take a percentage off of homework/tests/projects for being handed in late.
    • In case of absence from class, parents are asked to communicate with tutors as a matter of common courtesy to explain the reason for their child's absence. An email, note, phone call, text, or in person conversation are all acceptable means of communicating a student's absence. In these instances, the tutor may provide an additional grace period beyond the one class day.
    • Long term assignments (assignments given to students at least two weeks in advance of their absence) and short term assignments are due the day of the student's return from an absence.
    • If a student is carrying an "F" grade for at least 2 quarters in a class, BTM will withdraw them from that class, but not their other "passing' classes.

     

  • Safety Policy

    Bridge Tutorial Ministries has a Child Protection Policy in place that all Tutors and Leadership Team Members have agreed to follow. Parents may request a copy of this policy.

    As an agreement with the host facility, students must be supervised by an adult at all times. BTM's Leadership Team agrees that this is for their safety and accountability, as well as importance for compliance with our liability insurance. Students may not wait for a ride outside alone or be outside for any reason without direct adult supervision. Students, who are not siblings with each other, may not gather in vehicles in the parking lot without direct adult supervision. Any student in a car should be so with the intention of leaving the BTM premises within several minutes. They may not be in a classroom or any room, aside from the restroom, alone or without the presence of a nearby supervising adult. There must be an adult close enough to see and hear the students for proper supervision.

    Students should not enter a room with only one adult present. Wait for another student or adult before entering.

    Electronics Policy

    • Electronic devices may be used while at BTM in the Student Center for brief texting and to listen to music via headphones/earbuds, but NOT in the classrooms, unless under the direction of the tutor.
    • Devices, including headphones/earbuds/airpods, may not be shared for any reason. Misuse of this privilege will result in disciplinary action and possible loss of use of the Student Center.
    • Cell phones are to be left on vibrate while at BTM and must remain stored while in the classroom.
    • Electronic devices/cell phones used to record, play video games, watch videos, access social media is prohibited at BTM. If used in any of the above manners, the device will be taken until a parent retrieves it upon pick up.
    • In order to keep the student center interactive, students may not be on their phones the entire period. The DP on duty will ask students to put their phones away and suggest an interactive activity if cell phone usage is excessive (ie, not taking a break after 5-10 minutes of usage.
    • If laptops are being used they must be visible to the DP at all times.
    • Reading for pleasure on Kindle or electronic reader devices is permitted in the Student Center.
    • BTM will not be held responsible for any lost, stolen or damaged electronic devices. Bring them at your
    • During class time, each tutor reserves the right to collect all student phones at the beginning of class and return them at the end of class.
    • It is possible that a parent or student may request to use a device to voice record class. The tutor must be notified prior to class being recorded. Should any other students not want their voices recorded they should notify their tutors and the leadership team at the beginning of the year. In this case the class discussions will not be recorded.

     

  • Student Center Policy

    • Uphold all items listed in the Student Expectations section of the Parent/Student Agreement while in the Student Center.
    • The Student Center may only be used for an equal number of periods that a student is registered for classes. For example, 2 classes = 2 periods in the Student Center.
    • Uphold all items listed in the Electronics Policy.
    • Respond immediately and respectfully to any requests that are made of you by the DP on duty, any tutor or LT member.
    • Assist in cleaning up the table that you use, as well as the surrounding floor areas, immediately after finishing your lunch/snack. Encourage everyone at the table to clean up immediately. If asked to clean up an item that is not yours, please cooperate immediately.
    • No loud talking, yelling, horse playing, throwing of food/water bottles/food wrappers or extensive physical contact while at BTM.
    • No videos/recordings of anyone or anything is permitted at BTM, unless under the supervision of a tutor.
    • Not adhering to any of the above guidelines could result in the loss of use of the Student Center and a parent's presence during a student's free periods.
    • Thorough completion of all Designated Parent Duty days are necessary for a family's use of the Student
  • Dress Code Policy

    Modest clothing is to be worn at all times while at BTM or BTM sponsored events including educational and parent led outings. The basic guideline is that all private body parts must be well covered while standing or moving around. Students must dress appropriately to show respect to others and to keep others from distraction by dressing modestly. If a student is in violation of these requests, they may be asked to leave the classroom or tutorial until the student is in compliance.

    Not Permitted:

    1. excessively tight or loose clothing

    2. excessively short clothing (see below for how to check length)

    3. exposed waistline, cleavage, or underwear

    4. excessively ripped clothing or see-through clothing - especially in areas that should be covered according to the basic guidelines mentioned above

    5. excessive dirt/grime on clothes

    6. strapless tops

    7. profanity or offensive advertisements

    Remember:

    • tops must have some sort of strap going over both shoulders (this does not include a bra strap or a jacket/sweater)
    • skirts and shorts hemline must fall below your fingertips when your arm is straightened out at your side, not just at your fingertips
    • that hemlines can ride up when you are moving around and that all private areas should be covered while in motion too
    • no hoods are to be worn on your head at all while in the host facility
    • no headphones, earbuds or airpods are to be in your ears at all while in class
    • ripped jeans must not be excessively ripped (see rule #4) and any rips need to be below your fingertips
    • leggings are excessively tight (see rule #1) so if you wear leggings as pants, cover your whole backside with your shirt/top/sweater- which should be well below your derriere 
    • if someone makes a fashion mistake, show respect and look away

    Dismissal and Parking Lot Policy

    • Students must wait in the foyer until their ride is actually on the host facility's premises. They may not wait on the walkway or in the parking lot, even if parents have let their students know that they are close by
    • If a student does not have a period 6 class, which begins at 2:30, he must promptly check out and must be picked up at 2:30 sharp, even if their sibling has a class after then.
    • Students are not allowed to be in the parking lot for any reason, unless they are getting into a vehicle and leaving the premises immediately
    • If older siblings are responsible for transporting BTM students, they must not linger in the parking lot long before that sibling's dismissal time, nor after that time.
    • No other BTM students are permitted to wait in cars with anyone except their parent or sibling
    • If a parent is waiting for a student to finish class, but has agreed to supervise one of their other children/other BTM students outside, the parent MUST sit/stand outside their vehicle and be in VERY close proximity to where their student is playing outside When that parent is ready to leave, he must ensure that the other students are securely with their own parent or back in the foyer
    • Once students check out and leave the foyer, they must take ALL of their belongings with them and will NOT be permitted back into the building Please arrange to use the bathroom facilities and bring extra water for the end of the day
    • In the event a family has an emergency and is unable to arrive promptly at the end of their student's last class, the parent is expected to inform the leadership team ASAP. Please consider getting to know other BTM parents as your first line of assistance when these situations arise.

    Virtual Learning Policy

    In order to maintain the integrity of the virtual classroom and minimize distractions, the following guidelines must be observed during all online courses with Bridge Tutorial Ministries:

    1. If cameras are on, students must be fully dressed and present in a well-lit room throughout the entire class period. Students should adhere to the BTM dress code while online. It will be up to the individual discretion of each tutor if they require cameras and audio to be on or off during class time.

    2. Students must sign in with their real name, first and last, and must not change their screen name during class

    3. Only registered students should appear on camera at any time during class.

    4. If online classes are recorded, parents and students agree to neither forward class recordings to non-class participants, nor post any class recordings outside of the virtual classroom for that course.

    5. Any virtual backgrounds that are used must be one solid color without images or text. If a solid virtual background is used, once selected it must not be changed during class time.

    6. Do not take any photos or screenshots of classmates or tutors.

    7. If you need to leave the screen for any reason during class, turn off your video until you return to the screen.

    It is extremely important that all parties involved understand these requirements. You and your students will be held accountable to this agreement and subsequent consequences as needed for failure to comply.

    Please list any special physical needs, medical conditions, regularly given medications or allergies that the adults in charge or medical professionals should be aware of if your student is in an emergency situation (including food allergies). Please describe CLEARLY the exact reaction he/she has to an allergen, when (if ever) the last allergic reaction occurred, and what procedures need to be taken to assist them if needed. I understand if my student requires an inhaler/Epipen that he/she is responsible for having it with him/her at all times. BTM staff are not trained to administer medication or work with special needs students. You are to communicate directly with your tutor specific information that will help your student be successful in the classroom. This form is for medical emergencies.

  • I give my permission for my student to participate in academic and extracurricular activities at Bridge Tutorial Ministries. In case of an emergency, I authorize any medical treatment and the use of this information form by medical personnel in my absence for the well-being of my student. In accordance with BTM's Liability Release Form, I agree to not hold liable Bridge Tutorial Ministries, the host facility and all its representatives, BTM leadership team, tutors and all parent volunteers treating my son/daughter from any injury or sickness occurring during the activities associated with the BTM tutorial program whether at the host facility or on a parental approved outing. I have read and am in agreement with all statements on this form:

  • COVID-19 WAIVER AND RELEASE 2022-2023

    Bridge Tutorial Ministries (hereinafter collectively referred to as "BTM") adopted the following guidelines and procedures ("General Rules") for the safety and health of students, tutors and families using facilities (hereinafter collectively referred to as "Facilities" Each participant must sign this form and follow the General Rules when using the Facilities. These General Rules are subject to change without notice. BTM reserves the right to terminate privileges for anyone who does not follow the General Rules herein and Rules posted at the Facilities. BTM reserves the right to close the Facilities entirely if it is determined that students are not following the General Rules or if it is determined that the Facilities should be closed due to general health and safety concerns.

    General Rules for Use of the Facilities:

    Participants must adhere to the following rules and guidelines when using the Facilities:

    -Participants must follow the facility and/ or county current mask mandates as informed and updated by the BTM LT.

    -Thoroughly wash and/or sanitize your hands upon entering and throughout use of the Facilities.

    -Thoroughly wash and/or sanitize all object(s) you bring into the Facilities area.

    -Participants LT/DP/ Tutors will be responsible for sanitizing touch surfaces. They will wipe down surfaces prior to another student utilizing the same surface.

    -Guests are NOT allowed within the Facilities, unless permitted for specific purposes by the BTM LT. Only current BTM families, Tutors, Designated Parents, Leadership Team members, and minors from the same household as a Tutor, Designated Parent or Leadership Team member may use the Facilities.

    -Participants are responsible for maintaining standard restroom cleanliness - toilets flushed, nothing left on floors or sink areas, etc leaving the restrooms in clean condition after use.

    -BTM will NOT be maintaining a Lost & Found. All items will be disposed of if not claimed by the end of the day.

    B. COVID 19 Inherent Dangers

    I acknowledge the contagious nature of COVID-19 and that it can be spread directly between participants using the Facilities. I am aware that frequently touched surfaces allow for the transfer of infected respiratory droplets and that COVID-19 can survive on those surfaces. I also understand that there is a risk of contracting COVID-19 by using the Facilities.

    I am aware that frequently touched surfaces include, but are not limited to, the following:

    -Light switches, doors and door handles.

    -Restroom faucets, sinks, soap, paper towel dispensers, toilet flush controls.

    -Stair railings.

    -Tables, chairs, lab equipment, writing utensils and electronics.

    -Drinking fountains (will be off limits)

    I understand and acknowledge that in the event of contracting COVID-19 or any other illness or injury, BTM and the host facility do not owe me a legal duty to take any action on my behalf.

    C. Assumption of Risk

    I certify that I am physically fit and suffer from no condition, impairment, disease, infirmity, or other illness (including COVID-19) that would endanger others or prevent me from using the Facilities. I acknowledge that given the inherent risk of contracting COVID-19, BTM has taken all reasonable and appropriate precautions to limit its transmission, and that despite taking these precautions, BTM cannot guarantee that COVID-19 will not be transmitted in its Facilities. I acknowledge that there will be no enforcers to ensure appropriate social distancing at all times, or to ensure that each person complies with the General Rules stated herein; however, tutors, DP and LT will be there to ensure that guidelines are being followed to the best of their abilities. I acknowledge there may be instances when appropriate social distancing is not being maintained, or other General Rules are not being followed, by those in the facility and that I am responsible to remove myself and/or my minor children if I should become uncomfortable for my/our health or safety.

    I hereby assume all risk of loss, danger, property damage or injury (including death) to myself and my minor children from COVID-19 infection or other illnesses or injuries arising from the use of the Facilities, and I hereby agree that I am solely responsible for any resulting illness or personal injury, including death, to myself and my minor children, as a result of using the Facilities.

    D. Waiver, Release and Indemnification

    In consideration of being able to use the Facilities during this time of the threat of COVID-19, I, for myself, my heirs, personal representatives and assigns, do hereby release, waive, discharge and covenant not to sue BTM its leadership team, tutors, volunteers, no other BTM families, the host facility's staff, church members or contractors (collectively referred to as "Released Parties") from any and all claims or liabilities, including but not limited to, illness, personal injury (including death), and property loss, which may result from my use of the Facilities or arising from the negligence or fault of the Released Parties. I further save and hold harmless and indemnify the Released Parties from any and all claims, actions, lawsuits, damages and liabilities, including attorney's fees, resulting directly or indirectly from my use of the Facilities, whether caused by the negligence of the Released Parties or otherwise, and to reimburse the Released Parties for any such expenses incurred by them from my use of the Facilities.

    PARENT/GUARDIAN WAIVER FOR MINORS (Under the age of 18): The undersigned parent(s) or legal guardian(s) do hereby represent that he/she has consented to his/her child's use of the Facilities, and has agreed on behalf of the child, to the terms of this waiver and release of liability and to save and hold harmless and indemnify the Released Parties from any and all claims, actions, lawsuits, damages and liabilities, including attorney's fees, resulting directly or indirectly from my use of the Facilities, whether caused by the negligence of the Released Parties or otherwise, and to reimburse the Released Parties for any such expenses incurred by them from the child's use of the Facilities.

    I HAVE READ THIS DOCUMENT AND UNDERSTAND ITS TERMS. BY SIGNING THIS DOCUMENT, I AM WAIVING ANY

    RIGHT I OR MY MINOR CHILDREN OR SUCCESSORS MIGHT HAVE TO BRING A LEGAL ACTION OR ASSERT A CLAIM 

    AGAINST BTM AND ITS RELEASED PARTIES FOR ANY INJURY OR DEATH SUSTAINED. I HAVE SIGNED THIS FREELY AND VOLUNTARILY.

    I expressly agree that the foregoing waiver and release of liability, indemnity agreement and assumption of risk is intended to be as broad and inclusive as is permitted by the law of the State of Maryland and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

    One Form Per Family


    While participating in the tutoring services organized by Bridge Tutorial Ministries and provided by independent contract tutors, whether at the main tutoring location of the host facility, or at an off-site location for an official Bridge Tutorial sponsored event, I agree for myself and for the members of my family to the following:  

    1. I agree to obey all posted rules and warnings and further agree to follow any oral instructions or directions given by Bridge Tutorial or the Tutors associated therewith.
    2. I recognize there may be certain inherent risks associated with the course activities, and I assume full responsibility for the personal injury to myself and any family members and further release and discharge Bridge Tutorial, the Tutors, the Leadership Team, the host facility church and its staff for injury, loss or damage arising out of my or my family’s use of their services or presence at the facilities, whether caused by fault of my own or that of my family, or other third parties.
    3. I agree to indemnify, hold harmless and defend Bridge Tutorial and the Tutors, the host facility and its staff against all claims, causes of action, damages, judgments, costs or expenses, including medical fees, attorneys fees and other litigation costs which may in any way arise from my or my family’s use of their services or presence at the facilities.
    4. I agree to pay for all damages caused by me or my family’s negligent, reckless, or willful actions.
    5. Any legal or equitable claim that may arise from participation with the Bridge Tutorial Ministries shall be resolved in accordance with the laws of the State of Maryland.

    I have read this document and understand it. I further understand that by signing this release, I voluntarily surrender certain legal rights.

  • BTM Financial Agreement 2022-2023

    One Form Per Family

    We agree to adhere to all Bridge Tutorial Ministries financial commitments written in the Financial Agreement below.

  • We understand that the Registration Fee for each student is nonrefundable and is due, via cash, check or PayPal( Friends and Family only option or pay the additional fee), prior to being able to register for classes. Facility Fees will be due April 22, 2022 and Student Center Fees will be due August 15, 2022, once meeting decisions have been determined. 

    We hereby agree to pay all tutors, who we register our student(s) to receive instruction from, directly for their services and any associated fees at the set amount established by the tutor and in accordance with the agreed payment schedule. If my payment(s) is late, (after the 10th of the month), I agree to add a $10 late fee/class/student payment to the owed amount.  I agree to pay the $10 late fee if my first payment or any subsequent payments are late.

    We also agree to pay the various fees incurred should we change or drop a class, or withdraw from the tutorial.  The fee to change or drop a class is $35/ class/student; the fee to withdraw from the tutorial is $50/student, both due to BTM via paypal, using BTMLeadershipteam@gmail.com.  

    Furthermore, it is recognized that if our student withdraws from the course(s) at any time or for any reason, between the dates of June 10 through December 10, we, the parents, agree to fully compensate the tutor for their services and any associated fees for the first 5 payment months. And accordingly, if he/she is withdrawn on any date after December 10, we, the parents, will fully compensate the tutor for their services and any associated fees for the last payment months of December through April. This includes a student’s withdrawal from class that has been requested by the administration or the tutor.

    Note: If a student is going to withdraw from the second semester of classes, the parent must tell BTM Administration and the student’s tutor in writing by December 10th. Any decision after this date will mean the family will pay for the full second semester of classes.

    Photo and Contact Release Agreement 2022-23

    One Form Per Family

  • The undersigned, hereby, states and agrees as follows: 

    Bridge Tutorial Ministries has my permission to include photographic images of me/my child/children in print, and/or computer-based materials including BTM’s website, promotional events, Facebook page, and yearbook. (Materials contained on the website are subject to U.S. copyright law: Title 17, United States Code.) 

    Photos will also be used in the creation of Photo Identification pages for the check in process on BTM days. The student’s name will be on the card, but no other identifying information (i.e., address or phone number) will accompany the images on the pages.

    My child’s voice may be recorded should another student request to make an audio recording of class.  My student will be notified of this possibility.

    Bridge Tutorial Ministries has my permission to use my family’s contact information (address, phone number, and email) to create a Family Contact Directory which will be available to other BTM families.

    No compensation has been received by me in exchange for giving my permission to use these materials.

  • Designated Parent Commitment 2022-2023

  • For Designated Parent Duty, I commit to:

    1. Sign up for the required number of full DP days and alternate days on Sign Up Genius,

    2. Fill in the google document on the website with my DP dates, as instructed in the Registration Procedures,

    3. Write the DP dates I sign up for on my calendar,

    4. Respond to the reminder email from BTM LT within 24 hours from when it was sent to confirm my ability to fulfill my commitment,

    5. Arrange my schedule accordingly so that I can thoroughly serve the full 7 hours that I am needed at BTM,

    6. Read the DP Guidelines, set up and break down procedures right before serving as DP, so I am very familiar with what is expected of me during my time on duty,

    7. Use the Sign Up Genius switch function appropriately, leaving adequate time to attempt to swap dates with another parent when necessary,

    8. Understand that the use of the alternate parent is ONLY for the BTM Leadership Team to utilize for last minute emergencies,

    9. Understand that my cell phone will be shared in the reminder email with the LT and the other DP on duty.

    If I fail to follow through with each of the above commitments, I understand that:

    1. Missing my DP date and/or not responding to the BTM LT reminder will require that I, upon each occurence, remain in the building at BTM, for the next BTM day for the entire time my student is in class and leave once my student's classes are completed. Additionally, I may be called upon to serve another duty day if needed.

    2. If I continue to miss my date and/or not respond to the email within the requested time parameters, I will lose the privilege of being able to register as a current family for the following school year, and will have some immediate limitations of using the Student Center during the current school year.
