This form is to be completed by a coach or teacher-in-charge ONLY, no later than Monday, March 28 at 11:59PM EST.

In Part 2 of this form, you will electronically sign and submit the "Agreement Regarding Participant Supervision" (one per school).

In Part 3, parents or guardians will sign three relevant forms, and you will upload one combined PDF for ALL your participating students' forms:

Part 2: Form Submission - "School Participant Supervision Agreement"

We require one electronic submission and one PDF submission from you. The first submission is the "Agreement Regarding Participant Supervision" -- to be signed by an authorized official of the school (one form per school). You can sign this electronically below.

AGREEMENT REGARDING PARTICIPANT SUPERVISION

My organization is participating in the Osterweis (“Program”). I understand and agree to the following terms for my organization’s participation in the Program:

1. My organization and its school or other supervising authority (collectively “Organization”) will establish, distribute, and enforce reasonable rules of behavior, safety, and supervision for adults and minors attending the Program under the auspices of my Organization (“Organization Members”).

2. Organization Members will adhere to all requirements imposed by my Organization regarding their behavior, safety, and supervision.

3. My Organization will obtain and have available during the Program any health forms necessary for the provision of medical treatment to Organization Members. Such forms shall include permission for Yale to seek emergency medical treatment for Organization Members.

4. Neither Yale University nor the Program (collectively “Yale”) will be responsible for the behavior, safety, or supervision of Organization Members.

5. My Organization releases Yale from all legal and financial responsibility for any harm that the Organization, Organization Members, or their property might suffer as a result of my Organization's failure to meet its obligations under this Agreement.

6. My Organization shall indemnify Yale against and hold Yale harmless from (that is to say, my Organization agrees to pay or reimburse Yale for) any costs, penalties, legal fees, or judgments that Yale has to pay related to my Organization’s failure to meet its obligations under this Agreement.

7. The laws of Connecticut shall govern and the courts of Connecticut shall interpret this Agreement.

Part 3: Individual Form Submission
The last submission is the combined "Permission for Online Learning," "Assumption of Risk," and "Use of Images" form -- to be signed by the student(s)'s parent or guardian.

1. Parents/guardians must PRINT the form and fill out sections by hand. No electronic signatures will be accepted for this form. ALL SECTIONS must be filled out. Any sections left blank voids the entire form.

2. Combine forms for ALL students into ONE PDF and upload this document below.

3. Name your PDF appropriately: Parent Release and Media Form: "(2/2) [SCHOOL NAME] - X Students - Individual Forms"
