By completing this form, I am submitting an appointment request. In response, I will receive an email/text with recommended services. In most cases, an in person or virtual consultation will be suggested before scheduling your first appointment to ensure that we are good fit, and to determine services that will your individual needs. Generally, new guests are able to be seen within 1-2 weeks of appointment submission request, but guests with limited availability may have to wait up to 4 weeks to be seen.

I acknowledge and understand that we have a strict 48 hour cancellation policy and if I do cancel my appointment with less than 48 hours notice, I will be charged a cancellation fee equal to 50% of services reserved.
