Employee of the Month Nomination Form
Any employee may nominate a CMC colleague as long as the nominee is a full-time or part time employee and not a temporary employee. Please take into consideration the employee’s performance overall but make special consideration for the month being nominated. We really want to know who your shining star is for that month. Requirements are as follows: 1) May not be self-nominated 2) Current status on all mandatory training 3) Must be a CMC employee for at least six months 4) No formal disciplinary action on file within 12-months of date of award 5) Meets or exceeds responsibilities of the job 6) Works safely at all times 7) Shows up on time and is prepared to start the day 8) Has a positive attitude 9) Is hungry to learn more and strives for consistent improvement 10) Sets an example for peers at work and off the job 11) Epitomizes the CMC mission statement and core values on our company website.
