    SH Summer Mission Trip to SOS in Memphis, TN, June 12-17, 2022.

    The health and safety of our students is our #1 priority.  In light of COVID-19 pandemic, we think it is important that you understand our efforts to manage your student's health and safety so that you can make an informed choice.  We are focused on taking all reasonable measure to prevent the spread of COVID-19 in our group. We will follow the regulations of the CDC, of St Louis County. Increase frequency measure for things such as wiping down common touch points, dining hall areas, and activity equipment.  Measures will be taken to monitor and address symptomatic students and staff by daily temperature checks and protocols to isolate, confirm, respond, and remove any student or staff member with suspected COVID-19.  This situation continues to change daily, and as such, we will adapt and adjust our protocols and procedures as we follow the guidance provided by the CDC and local health departments, in our efforts to help keep our students, staff, and families safe.   Ultimately, the choice for you student to attend summer camp is a personal one, and you are in control.

    I also authorize the Released Parties to arrange for transportation of my child as deemed appropriate in their discretion. If the child is less than 18 years of age, the parent(s) having legal custody and/or the legal guardian(s) of the Volunteer also hereby release, forever discharge and hold harmless the Released Parties from any liability, claim, demand and action whatsoever brought by such volunteer or on his/her behalf which arises or may hereafter arise on account of the decision by any representative or agent of the Released Parties to exercise the power to transport, as set forth in the Parental Authorization for travel with a Minor Child.

    I understand that FIRST EVANGELICAL FREE CHURCH carries medical and hospitalization insurance coverage which, consistent with the exclusions, limitations and terms thereof, may provide benefits over and above any personal medical and hospitalization coverages available to my family. I understand that any personal medical and hospitalization insurance available to my family will provide primary coverage and the ministry's medical and hospitalization coverage (subject to the exclusions, limitations and provisions in the ministry's policy) may provide secondary or excess coverage. I agree to apply first for benefits from the personal hospitalization and medical coverages available to my family, if any, before applying for benefits that may be available from the ministry's medical and hospitalization coverage. 

    I hereby consent and give permission to the ministry's sponsor or any adult counselor acting on behalf of the ministry with respect to the Activity, as agent for me, to consent to any X-ray examination; injections; anesthesia; medical, dental or surgical diagnosis and treatment; and hospital care and treatment advised and supervised by a physician, surgeon, or dentist (as appropriate) licensed to practice under the laws of the state where the services are rendered, either as an outpatient or in any hospital. To the best of my knowledge, I have listed above all of my medical allergies, medications being taken, medical problems and other pertinent information.
