I give permission for Inka Studios LTD, Derby, Kedzink LTD, Spudwop LTD, LightningFox Tattoos LTD and Jade Tattoo LTD to obtain my: name, address, date of birth, mobile phone number, home phone number and email address. I fully understand that they will keep this personal information safe on record. I fully understand that Inka Studios LTD, Derby, Kedzink LTD, Spudwop LTD, LightningFox Tattoos LTD and Jade Tattoo LTD will use my full name and address as a form of identification. I fully understand that Inka Studios LTD, Derby, Kedzink LTD, Spudwop LTD, LightningFox Tattoos LTD and Jade Tattoo LTD will use my date of birth to check that I am of the legal age to obtain a tattoo in accordance with the tattooing of minors act 1969. I fully understand that lnka Studios LTD, Derby, Kedzink LTD, Spudwop LTD, LightningFox Tattoos LTD and Jade Tattoo LTD will use my mobile phone number/s, home phone number/s and email address/s to contact me about anything regarding my tattoo only. I fully understand that only Inka Studios LTD, Derby, Kedzink LTD, Spudwop LTD, LightningFox Tattoos LTD and Jade Tattoo LTD will have access to my personal data and no third party shall be provided or have access to any personal information. I fully understand that I have the right to withdraw any personal information at any point in time.

I can confirm that I have read all of the above carefully and I give consent for Inka Studios LTD, Derby, Kedzink LTD, Spudwop LTD, LightningFox Tattoos LTD and Jade Tattoo LTD to obtain my personal data and to keep it safe in accordance with the general data protection regulation (GDPR) (EU) put in place on 25th of May 2018.

I UNDERSTAND THAT AS A PERSON UNDER THE AGE OF 21, I MUST BRING A PROOF OF IDENTIFICATION TO MY TATTOO APPOINTMENT FOR INKA STUDIOS LTD, DERBY, KEDZINK LTD, SPUDWOP LTD, LIGHTNINGFOX TATTOOS LTD AND JADE TATTOO LTD TO CHECK THAT I AM OF THE LEGAL AGE TO OBTAIN A TATTOO IN ACCORDANCE WITH THE TATTOOING OF MINORS ACT 1969.
I UNDERSTAND THAT FAILURE TO PROVIDE A FORM OF IDENTIFICATION ON THE DAY OF MY APPOINTMENT WILL RESULT IN ME NOT BEING TATTOOED. I UNDERSTAND THAT THE FORM OF IDENTIFICATION MUST CONTAIN A PHOTO OF ME AS WELL AS CONTAINING MY DATE OF BIRTH.
(EXAMPLES OF ACCEPTABLE FORMS OF IDENTIFICATION INCLUDE; PASSPORTS, DRIVING LICENSES, NATIONAL IDENTITY CARDS, ETC. WE DO NOT ACCEPT FORMS OF IDENTIFICATION THAT INCLUDE; B-LINE CARDS, COLLEGE/UNIVERSITY CARDS, ETC.

I CAN CONFIRM THAT I HAVE READ ALL OF THE ABOVE CAREFULLY AND I GIVE CONSENT FOR INKA STUDIOS LTD, DERBY, KEDZINK LTD, SPUDWOP LTD, LIGHTNINGFOX TATTOOS LTD AND JADE TATTOO LTD TO OBTAIN A PHOTO-COPY OF MY FORM OF IDENTIFICATION ON THE DAY OF MY TATTOO APPOINTMENT IN ACCORDANCE WITH THE GENERAL DATA PROTECTION REGULATION (GDPR) (EU) PUT IN PLACE ON 25TH OF MAY 2018.

I have been clearly informed and I fully understand that all deposits and early payments paid to Inka Studios Ltd, Kedzink LTD, Spudwop LTD, LightningFox Tattoos LTD and Jade Tattoo LTD prior to any appointments are non-refundable. In the event of non-confirmation the appointment will be cancelled and the deposit forfeited.
I have been clearly informed and fully understand that I will be contacted prior to my appointment to confirm my attendance. Failure to contirm will result in my appointment being cancelled, and i fully understand if i cancel/rearrange more than 3 consecutive appointments, Inka Studios LTD, Kedzink LTD, Spudwop LTD, LightningFox Tattoos LTD and Jade Tattoo LTD reserve the right to cancel my future appointment/s and void my deposit/s
It is my responsibility to provide Inka Studios Ltd, Kedzink LTD, Spudwop LTD, LightningFox Tattoos LTD and Jade Tattoo LTD with correct and up to date contact information. Inka Tattoos Ltd will not be held responsible for incorrect contact details, or contact details that are no longer in use.
