Cheryl House provides community-based case management, resource navigation, and peer support to help individuals connect with local resources and specialized services. Based on your individual needs assessment, we will work with you to develop a personalized action plan and identify resources that may help address your needs. Completed intake forms are reviewed and responded to on Saturdays. If you need immediate resource assistance, please contact our Peer Support Specialist at 773-413-9809. Please note that response times may vary. Our services are made possible through the support and care of friends, family, neighbors, and community providers.
