I acknowledge that I am choosing to consume my placenta based on my own personal beliefs and that no specific benefits or effects are promised. I understand that my placenta has been handled and encapsulated acording to OSHA standards, and has been cleaned, dehydrated and put onto pill form in a sanitary work space in Melanie Belk's home. Upon receiving my placenta capsules and/or tincture, I waive any and all rights to hold Melanie Belk or Labor of Love responsible for any undesired effect of consuming the capsules. I do not hold Melanie Belk responsible or liable for any transport midhap that is beyond her control (e.g. car accident or detainment).
