Data: The applicant will submit timely and accurate project evaluation data as requested. This includes monthly Suicide Ideation Referral forms (SIRF) and end-of-year surveys and reports by Peer Leaders, school staff, and Adult Advisors. Funding for the second half of the grant will be awarded after ample end-of-year data collection.
Eligibility: The applicant is an 'eligible entity' for the Idaho Lives Project as a public Idaho junior high/middle school or high school.
Coordinated Approach: The school’s activities to be implemented will take into consideration other educational agency and/or community programs in order to assure a coordinated approach and to avoid duplication of effort.
Training Facility: A safe and easily accessible facility that meets Sources of Strengthguidelines will be provided for Gatekeeper, Adult Advisor and Peer Leader trainings.
Adult Advisors: The applicant will identify and support four to ten Adult Advisors from the school or community who meet Sources of Strength guidelines for Adult Advisors and who reflect the diversity of the school as a whole. These Adult Advisors will participate in two days of trainings and facilitate ongoing Sources of Strength activities with school Peer Leader team (approximately 4-5 hours per month).
Peer Leaders: The applicant will nominate and/or retain a diverse student Peer Leader teams using Sources of Strength selection procedures. The applicant will allow Peer Leaders to conduct program meetings and implement program activities/campaigns during regular school hours, rarely using lunchtime. Adult Advisors will assist with and oversee activities/campaign implementation.
Parental Consent: The applicant will obtain active parental consent for Sources of Strength Peer Leaders before and allow all Peer Leaders to participate in trainings.
Materials Requirements: All materials associated with the program will include the Idaho Lives Project logo and funding statement to be provided by ILP.
Key Personnel Changes: Changes in administrator or designated school Sources of Strength contact or co-leads from the date of this document through the end of the 2022/2023 school year will be reported to the SDE.
Legal Authority: The applicant has the legal authority to conduct all activities proposed to be funded under this project.
Deficiencies: The applicant will request support to correct/address deficiencies in program operations, or any program issues that are identified through Idaho Lives Project team or State Department of Education State Coordinator for Youth Suicide Prevention and Safety Coordination.
